中文摘要: 目的分析18F-氟代脱氧葡萄糖(18F-FDG)正电子发射断层扫描/计算机断层扫描(PET/CT)检测代谢肿瘤体积(MTV)和病灶糖酵解总量(TLG)在肺癌早期诊断及淋巴结转移评估中的价值,为临床决策提供指导。方法纳入2022年4月至2025年4月安徽医科大学附属安庆市立医院收治的78例疑似肺癌患者,入院1周内接受18F-FDG PET/CT检测,依据肺组织活检结果将78例患者分为肺癌组(n=58)和良性病变组(n=20),同时根据18F-FDG PET/CT检测结果将58例肺癌患者分为转移组(n=11)和未转移组(n=47)。比较肺癌组与良性病变组、转移组与未转移组18F-FDG PET/CT参数,采用受试者工作特征(ROC)曲线评估18F-FDG PET/CT参数对肺癌早期诊断、淋巴结转移的预测价值。结果肺癌组最大标准摄取值(SUVmax)、MTV、TLG高于良性病变组(P<0.05)。ROC曲线显示,SUVmax、MTV、TLG在肺癌早期诊断的曲线下面积(AUC)分别为0.888、0.840、0.916,灵敏度分别为91.38%、63.79%、91.38%,特异度分别为75.00%、95.00%、75.00%(P<0.01)。转移组MTV、TLG高于未转移组(P<0.05)。ROC 曲线显示,MTV和TLG诊断肺癌患者淋巴结转移的AUC 分别为0.737、0.731,灵敏度分别为45.45%、54.55%,特异度分别为100.00%、91.49%(P<0.05)。结论18F-FDG PET/CT检测的MTV和TLG水平,对肺癌的早期诊断及淋巴结转移均有较高的预测价值。

Abstract:Objective To analyze the application value of metabolic tumor volume (MTV) and total lesion glycolysis (TLG) of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT) in early diagnosis of lung cancer and assessment of lymph node metastasis, and to provide aguidance for clinical treatment. Methods A total of 78 patients who were suspected of lung cancer and admitted to Anqing Municipal Hospital Affiliated to Anhui Medical University from April 2022 to April 2025 were selected as the study objects. All of them received 18F-FDG PET/CT examination within 1 week after admission. Based on the results of lung biopsy, the 78 patients were divided into lung cancer group (n=58) and benign lesion group (n=20). Based on 18F-FDG PET/CT results, the 58 patients with lung cancer were divided into metastasis group (n=11) and non-metastasis group (n=47). 18F-FDG PET/CT parameters were compared between the lung cancer group and the benign lesion group, as well as between the metastasis group and the non-metastasis group. The receiver operating characteristic (ROC) curves were used to assess the value of 18F-FDG PET/CT parameters in early diagnosis of lung cancer and assessment of lymph node metastasis. Results Standardized uptake value maximum (SUVmax), MTV, and TLG in the lung cancer group were higher than those in the benign lesion group (P<0.05). The ROC curve showed that the AUC of SUVmax, MTV, and TLG in early diagnosis of lung cancer were 0.888, 0.840, and 0.916, respectively. The sensitivity were 91.38%, 63.79%, and 91.38%, respectively, and the specificity were 75.00%, 95.00%, and 75.00%, respectively (P<0.01). MTV and TLG in the metastasis group were greater than those in the non-metastasis group (P<0.05). The ROC curve showed that the AUC of MTV and TLG for diagnosing lymph node metastasis in lung cancer patients were 0.737 and 0.731, respectively. The sensitivities were 45.45% and 54.55%, and the specificities were 100.00% and 91.49%, respectively (P<0.05). Conclusion MTV and TLG of 18F-FDG PET/CT demonstrate high value in early diagnosis of lung cancer and assessment of lymph node metastasis.
