中文摘要: 目的比较光子计数探测器CT(PCD-CT)图像与传统能量积分探测器CT(EID-CT)图像在胸部低剂量CT(LDCT)中的图像质量,探究PCD-CT是否能获得更好的诊断图像。方法连续纳入76例于2024年2月至3月在江苏省省级机关医院放射科行PCD-CT胸部LDCT检查患者(PCD-CT组),应用倾向性评分匹配,按照年龄、性别、容积CT剂量指数(CTDIvol)匹配于2023年9月至12月在江苏省省级机关医院放射科行EID-CT胸部LDCT检查患者76例(EID-CT组)。主观等级评价图像呼吸伪影和肺小叶间隔锐利度。客观测量图像的噪声、信噪比(SNR)。结果呼吸伪影等级评分PCD-CT组和EID-CT组差异无统计学意义(P=0.127)。PCD-CT组肺小叶间隔锐利度等级评分明显优于EID-CT组(P<0.01)。PCD-CT组肺组织图像噪声明显小于EID-CT组[(61.6±7.7)HU vs(76.7±8.2)HU,t=11.844,P<0.01],PCD-CT 组肺组织SNR 明显大于EID-CT 组(15.1±2.0 vs 11.1±1.5,t=13.481,P<0.01)。两组纵隔组织噪声及SNR差异无统计学意义(P>0.05)。结论PCD-CT的图像质量优于EID-CT。在胸部LDCT的成像中,PCD-CT在提高图像质量方面显示出巨大的潜力。
Abstract:Objective To compare the image quality of photon-counting detector CT (PCD-CT) images with that of conventional energy-integrating detector CT (EID-CT) images in chest low-dose CT (LDCT) imaging, and to investigate whether PCD-CT can achieve better diagnostic imaging. Methods A total of 76 patients who underwent PCD-CT chest LDCT at the Department of Radiology, Jiangsu Province Official Hospital from February to March 2024 were continuously enrolled (PCD-CT group). Propensity score matching was applied to match 76 eligible EID-CT patients (EID-CT group) who underwent EID-CT chest LDCT at the Department of Radiology, Jiangsu Province Official Hospital from September to December 2023 according to age, gender, and volume CT dose index (CTDIvol). Subjective grading was performed to evaluate image respiratory artifacts and sharpness of interlobular septa. Objective measurements included image noise and signal-to-noise ratio (SNR). Results There was no significant difference in the grading score of respiratory artifacts between the PCD-CT group and the EID-CT group (P=0.127). The grading score of sharpness of interlobular septa in the PCD-CT group was significantly better than that in the EID-CT group (P<0.01). The image noise of lung tissue in the PCD-CT group was significantly lower than that in the EID-CT group [ (61.6±7.7) HU vs (76.7±8.2) HU, t=11.844, P<0.01], and the SNR in the PCD-CT group was significantly higher than that in the EID-CT group (15.1±2.0 vs 11.1±1.5, t=13.481, P<0.01). No significant difference showed in mediastinal tissue noise and SNR between the two groups (P>0.05). Conclusion The image quality of PCD-CT is superior to EID-CT. In chest LDCT imaging, PCD-CT shows great potential in improving image quality.
