中文摘要: 目的 探讨骨代谢及骨密度相关指标与骨量减少患者中医证型的相关性。方法 回顾性选取2021年1 月至2024年12月南京鼓楼医院中医科收治的骨量减少患者107例为研究对象。收集患者骨代谢及骨密度相关 指标。对患者进行中医辨证分型,分为肾虚证组(n=45)及非肾虚证组(n=62)。比较两组患者骨代谢及骨密 度异常程度以及可能的影响因素采用多因素 logistic 回归分析骨量减少肾虚证的影响因素;采用 Pearson 或 Spearman分析年龄、身体质量指数(BMI)与骨代谢或骨密度指标的相关性。结果 Logistic回归分析显示,血清 25?羟维生素D水平低、腰椎2骨密度T值及腰椎3骨密度T值低是骨量减少患者发生肾虚证的危险因素(P<0.05)。 Pearson相关性分析显示,年龄与股骨颈骨密度T值呈负向直线相关(r=-0.206,P=0.033);Spearman相关性分析显示,BMI与下肢全部骨密度T值、血清Ⅰ型胶原羧基末端肽β特殊序列(β?CTX)与血 清Ⅰ型前胶原氨基端前肽(PINP)、25 羟-维生素 D 与腰椎 1 骨密度 T 值分别呈正向直线相关(r=0.308,P= 0.001;r=0.703,P<0.001;r=0.209,P=0.030);β? CTX 与血清 25-羟维生素 D 呈负向直线相关(r=-0.212,P= 0.029)。结论 血清25?羟基维生素D、腰椎2骨密度T值及腰椎3骨密度T值与骨量减少肾虚证发生存在一定 相关性。年龄、骨代谢指标与骨密度之间存在一定相关性。

Abstract:Objective To investigate the correlation between bone metabolism and bone mineral density(BMD)related indicators and traditional Chinese medicine(TCM)syndrome patterns in patients with osteopenia. Methods A total of 107 patients with osteopenia admitted to the Department of TCM at Nanjing Drum Tower Hospital from January 2021 to December 2024 were enrolled as study subjects. Data on their bone metabolism and BMD- related indicators were collected. Patients were classified into a kidney deficiency syndrome group(n=45)and a non- kidney deficiency syndrome group(n=62)based on TCM pattern differentiation. The severity of abnormalities in bone metabolism and BMD,along with potential influencing factors,were compared between the two groups Multivariate logistic regression was used to analyze the influencing factors of kidney deficiency syndrome with osteopenia. Pearson or Spearman correlation analyses were used to assess the associations between age,body mass index(BMI),and bone metabolism or BMD indicators. Results Logistic regression analysis revealed that low serum 25?hydroxyvitamin D levels,low T?score for lumbar spine L2 BMD,and low T?score for lumbar spine L3 BMD were risk factors for developing kidney deficiency syndrome in patients with osteopenia(P<0.05). Pearson correlation analysis showed a negative linear correlation between age and femoral neck T- score(r=-0.206,P=0.033). Spearman correlation analysis indicated there were the positive linear correlations between BMI and lower limbs total BMD T?score,between serum β?crosslinked C?telopeptide of type I collagen(β?CTX)and procollagen type I N-terminal propeptide(PINP), between serum 25?hydroxyvitamin D and lumbar spine L1 BMD T- score(r=0.308,P=0.001;r=0.703,P<0.001;r=0.209,P=0.030),while a negative linear correlation between between β?CTX and serum 25? hydroxyvitamin D(r=-0.212,P=0.029). Conclusion There is a certain correlation between serum 25?hydroxyvitamin D levels,lumbar spine L2 T?score,lumbar spine L3 T?score,and the occurrence of Kidney deficiency syndrome in patients with osteopenia. Correlations also exist between age,bone metabolism indicators,and BMD.
