3D打印建模下超细经皮肾镜与传统经皮肾镜手术在<4 cm肾鹿角形结石治疗中的应用比较

中文摘要: 目的 比较3D打印建模下超细经皮肾镜、传统经皮肾镜手术治疗<4 cm肾鹿角形结石的穿刺成功率、创伤反应和复发率。方法 前瞻性选择江南大学附属中心医院泌尿外科2022年1月至2023年12月就诊的102例<4 cm肾鹿角形结石患者,遵循随机数字表法将其分为两组,各51例,传统组给予传统经皮肾镜手术治疗,3D组给予3D打印建模下超细经皮肾镜手术治疗,比较两组一次穿刺成功率、一期结石清除率、手术指标、肾功能指标[血肌酐(Scr)、尿素氮(BUN)]、创伤反应指标[去甲肾上腺素(NE)、皮质醇(Cor)、白细胞介素-8(IL-8)、C反应蛋白(CRP)]、并发症总发生率和复发率。结果 3D 组一次穿刺成功率(100.00% vs 88.24%,χ2=4.427,P<0.05)、一期结石清除率(98.04% vs 82.35%,χ2=7.096,P<0.05)较传统组更高。3D组手术时间、肾造瘘管拔除时间、住院时间均较传统组更短(P<0.05),3D 组术中失血量较传统组更低(P<0.05)。术后 3 d,两组血清 Scr、BUN水平均较术前降低,3D组血清Scr、BUN水平与传统组比较差异无统计学意义(P>0.05)。术后3 d,两组血清 NE、Cor、IL-8、CRP 水平均较术前增高,且 3D 组血清 NE、Cor、IL-8、CRP 水平均较传统组更低(P<0.05)。3D 组复发率较传统组更低(3.92% vs 17.65%,χ2=4.993,P<0.05)。结论 3D打印建模下超细经皮肾镜手术在<4 cm肾鹿角形结石治疗中,一次穿刺成功率、一期结石清除率较高,可有效改善患者肾功能,减轻创伤应激反应,降低复发率,具有手术时间短、术中失血量少、恢复快等优点。
Abstract:Objective To compare the puncture success rate,trauma response,and recurrence rate between ultra -fine percutaneous nephrolithotomy under 3D printing modeling and traditional percutaneous nephrolithotomy in the treatment of <4 cm staghorn kidney stones. Methods A total of 102 patients with <4 cm staghorn kidney stones treated in the Department of Urology,Jiangnan University Medical Center from January 2022 to December 2023 were prospectively selected and randomly divided into two groups,with 51 cases each. The traditional group was treated with traditional percutaneous nephrolithotomy,while the 3D group was treated with ultra -fine percutaneous nephrolithotomy under 3Dprinting modeling. The one-time puncture success rate,one-stage stone clearance rate,surgical indicators,renal function indicators[serum creatinine(Scr)and blood urea nitrogen(BUN)],trauma response indicators[norepinephrine(NE),cortisol(Cor),interleukin -8(IL -8)and C - reactive protein(CRP)],total incidence of complications,and recurrencerate were compared between the two groups. Results The one - time puncture success rate(100.00% vs 88.24%,χ2=4.427,P<0.05),one-stage stone clearance rate(98.04% vs 82.35%,χ2=7.096,P<0.05)in the 3D group were higher than those in the traditional group. The operation time,nephrostomy tube removal time,and hospital stay in the 3D group were shorter than those in the traditional group(P<0.05),and the blood loss during surgery in the 3D group was lower than that in the traditional group(P<0.05). Three days after surgery,the levels of serum Scr and BUN in both groups decreased compared with those before surgery,and there was no significant difference in the levels of serum Scr and BUN between the3D group and the traditional group(P>0.05). Three days after surgery,the levels of serum NE,Cor,IL-8,and CRP in both groups increased compared with those before surgery,and the levels of serum NE,Cor,IL -8,and CRP in the 3D group were lower than those in the traditional group(P<0.05). The recurrence rate in the 3D group was lower than that in the traditional group(3.92% vs 17.65%,χ2=4.993,P<0.05). Conclusion In the treatment of <4 cm staghorn kidney stones,ultra-fine percutaneous nephrolithotomy under 3D printing modeling has a higher one-time puncture success rate and one-stage stone clearance rate,can effectively improve patient renal function,reduce trauma stress response,and lower the recurrence rate,with advantages such as shorter operation time,less blood loss during surgery,and faster recovery.
