创伤严重程度评分联合乳酸对创伤失血性休克患者预后的预测价值

Predictive value of injury severity score combined with lactate for prognosis in patients with traumatic hemorrhagic shock

中文摘要: 目的 探讨影响创伤失血性休克患者预后的独立危险因素,评估创伤严重程度评分(ISS)与乳酸的预测价值。方法 回顾性分析2023年1月至2025年5月苏州大学附属无锡九院急诊抢救室收治的114例创伤失血性休克患者,根据患者入院28d的存活情况分为生存组(96例)与死亡组(18例)。比较两组性别、年龄、ISS、受伤机制、急诊机械通气、人纤维蛋白原使用情况、血制品输注情况、受伤至输血时长、血红蛋白、血细胞比容、血小板计数、凝血酶原时间、活化部分凝血活酶时间、国际标准化比值、纤维蛋白原、乳酸、血钙浓度、细胞外液剩余碱。采用多因素logistic回归分析确定独立危险因素,并通过受试者工作特征(ROC)曲线评估ISS、乳酸单独及联合应用对患者预后的预测效能。结果 多因素logistic回归分析结果显示,高ISS(OR=1.127,95%CI:1.029~1.235)、高乳酸(OR=1.575,95%CI:1.140~2.175)是创伤失血性休克患者死亡的独立危险因素,急诊机械通气(OR=0.066,95%CI:0.008~0.541)是创伤失血性休克患者死亡的独立保护因素(P<0.05)。ROC曲线分析表明,ISS与乳酸联合预测效能最高[曲线下面积(AUC)=0.904],乳酸和ISS单独预测的AUC分别为0.878和0.873。结论 入院时ISS与乳酸联合检测对患者预后具有良好的预测价值,有助于早期识别高危患者并指导临床干预。
Abstract:Objective To explore the independent risk factors affecting the prognosis of patients with traumatic hemorrhagic shock and to evaluate the predictive value of the injury severity score(ISS)and lactate. Methods A retrospective analysis was conducted on 114 patients with traumatic hemorrhagic shock admitted to the emergency resuscitation room of Wuxi Ninth People’s Hospital from January 2023 to May 2025. Patients were divided into the survival group(n=96)and the death group(n=18)based on their survival status at 28 days after admission. The two groups were compared in terms of gender, age, ISS, injury mechanism, emergency mechanical ventilation, use of human fibrinogen, blood product transfusion, time from injury to transfusion, hemoglobin, hematocrit, platelet count, prothrombin time, activated partial thromboplastin time, international normalized ratio, fibrinogen, lactate, blood calcium concentration, and extracellular fluid base excess. Multivariate logistic regression was used to identify independent risk factors, and receiver operating characteristic(ROC)curve analysis was performed to evaluate the predictive efficacy of ISS and elevated lactate individually and in combination for patient’s?? prognosis. Results Multivariate logistic regression analysis showed that ISS(OR=1.127, 95%CI:1.029-1.235)and high lactate(OR=1.575, 95%CI:1.140-2.175)were independent risk factors for the prognosis of patients with traumatic hemorrhagic shock, and emergency mechanical ventilation(OR=0.066, 95%CI:0.008-0.541)was an independent protective factor for the prognosis of patients with traumatic hemorrhagic shock(P<0.05). ROC curve analysis indicated that the combination of ISS and lactate had the highest predictive efficacy[area under curve(AUC)=0.904], followed by lactate alone(AUC=0.878)and ISS alone(AUC=0.873). Conclusion The combined detection of ISS and lactate at admission demonstrates good predictive value for patient prognosis, which aids in early identification of high risk patients and guides clinical intervention.
