中文摘要: 肝细胞癌(HCC)是全球范围内最常见的恶性肿瘤之一,其恶性程度高,治疗难度大,且预后差。嵌合抗原受体T细胞(CAR-T)疗法作为一种新型治疗手段已发展至第五代,在原发性肝癌治疗领域发展潜力巨大。磷脂酰肌醇蛋白聚糖 3(GPC3)在HCC中特异性高表达,且在肝癌早期具有较高检出率,使其成为CAR-T治疗HCC的理想靶点。然而,该疗法仍面临脱靶效应、肿瘤微环境抑制以及CAR-T耗竭等挑战。本文聚焦于CAR-T与新技术的联合应用及新药研发进展,全面综述了近年来国内外学者为增强其治疗效果的各项研究工作。
Abstract:Hepatocellular carcinoma(HCC)is one of the most prevalent malignant tumors globally,characterized by its high degree of malignancy,challenging treatment options,and poor prognosis. Chimeric antigen receptor T cell(CAR-T)therapy,as an innovative therapeutic strategy,has advanced to the fifth generation and demonstrates significant potential for the treatment of primary liver cancer. Glypican-3(GPC3),which is specifically overexpressed in HCC with a high detection rate even in the early stages of liver cancer,serves as an ideal target for CAR-T therapy. Nevertheless,this approach still encounters obstacles such as off-target effects,suppression by the tumor microenvironment,and CAR-T exhaustion. This article emphasizes the synergistic application of CAR-T with emerging technologies and the advancements in new drug development,providing a comprehensive review of the research endeavors conducted by scholars both domestically and internationally in recent years to enhance the therapeutic efficacy of CAR-T therapy.
