老年骨质疏松症患者血清LECT2水平及其预测价值

Serum LECT2 levels and its predictive value in elderly patients with osteoporosis

中文摘要: 目的 探讨血清白细胞衍生趋化因子2(LECT2)水平与骨质疏松症的相关性,评估其作为潜在诊断标志物的价值。 方法 纳入2023年6月至2024年10月南京浦口中医院收治的160例骨质疏松症患者(骨质疏松症组)及160例同期健康体检者(对照组)作为研究对象,比较两组下述指标:血清 Ⅰ型胶原羧基肽β特殊序列(β -CTX)、骨钙素(OCN)、Ⅰ型前胶原N端前肽(P ⅠNP)、25-羟维生素D[25(OH)D]等骨代谢指标,腰椎及股骨颈骨密度(BMD)以及血清LECT2水平。通过受试者工作特征(ROC)曲线分析评估相关指标的诊断效能,并采用Pear-son/Spearman相关性分析评估LECT2与骨代谢标志物及BMD的相关性。 结果 骨质疏松症组β-CTX、OCN、P Ⅰ NP及LECT2水平高于对照组(P<0.05),而25(OH)D水平及腰椎及股骨颈BMD值低于对照组(P<0.05)。ROC 曲线分析显示,LECT2对骨质疏松症具有较高的诊断效能,曲线下面积(AUC)为0.773,联合β-CTX、25(OH)D及 BMD指标后,诊断效能进一步提升(AUC=0. 843)。相关性分析表明,LECT2水平与β-CTX、OCN及PⅠNP呈显著正相关(P<0.01),与25(OH)D、腰椎及股骨颈BMD呈显著负相关(P<0.01)。 结论 骨质疏松症患者血液中 LECT2水平显著升高,且与骨代谢标志物及BMD存在显著相关性。LECT2联合传统骨代谢指标和BMD测量可显著提高骨质疏松症的早期诊断效能。
Abstract:Objective To investigate the correlation between serum leukocyte cell - derived chemotaxin 2(LECT2) levels and osteoporosis,and to evaluate its potential value as a diagnostic biomarker. Methods A total of 160 patients with osteoporosis(osteoporosis group)and 160 healthy individuals who underwent physical examinations during the same period(control group)at Nanjing Pukou Hospital of Traditional Chinese Medicine from June 2023 to October 2024 were enrolled. The following indicators were compared between the two groups: bone metabolism indicators such as serum levels of β-C-terminal cross-linked telopeptides of type Ⅰcollagen(β-CTX),osteocalcin(OCN),procollagen type Ⅰ N-terminal propeptide(P ⅠNP),25-hydroxyvitamin D[25(OH)D],as well as bone mineral density(BMD)of the lumbar spine and femoral neck,and serum LECT2 levels. The diagnostic efficacy of these indicators was assessed using receiver operating characteristic(ROC)curves,and the correlations of LECT2 levels with bone metabolic markers and BMD were evaluated using Pearson or Spearman correlation analysis. Results Compared with the control group,serum.β -CTX,OCN,P ⅠNP,and LECT2 levels were significantly higher in the osteoporosis group(P<0.05),whereas 25(OH)D levels and BMD values of the lumbar spine and femoral neck were significantly lower(P<0.05). ROC curve analysis showed that LECT2 had a high diagnostic efficacy for osteoporosis,with an area under the curve(AUC)of 0.773. When combined with β - CTX,25(OH)D,and BMD,the diagnostic efficacy was further improved(AUC=0. 843). Correlation analysis revealed that LECT2 levels were significantly positively correlated with β-CTX,OCN,and PⅠNP(P<0.01),and significantly negatively correlated with 25(OH)D,lumbar spine BMD,and femoral neck BMD(P<0.01). Conclusion Serum LECT2 levels are significantly elevated in patients with osteoporosis and are strongly correlated with bone metabolic markers and BMD. LECT2,in combination with conventional bone metabolic indicatorsand BMD measurements,can significantly improve the early diagnostic efficacy of osteoporosis.
