神经肌肉电刺激预防重症患者深静脉血栓的血流动力学时间变化特点

中文摘要: 目的 探讨在重症患者深静脉血栓(DVT)形成预防中应用神经肌肉电刺激(NMES)后血流动力学指标随时 间的变化情况。方法 选取2024年6月至2024年8月山东第一医科大学第一附属医院的40例重症监护病房(ICU) 患者为研究对象。分别于应用NMES前,应用NMES后30 min、1 h、1.5 h、2 h,以及停用NMES后30 min、1 h采用多普 勒超声采集患者腘静脉的血流动力学指标[峰值血流速度(PFV)、血流量、血管直径],分析其时间变化规律。 结果 40例患者不同时间点应用NMES,腘静脉PFV、血管直径和血流量总体之间差异有统计学意义(χ2 =179.767, F=92.058,F=196.013,P<0.01);不同时点比较显示,应用NMES 30 min、1 h、1.5 h、2 h时的PFV、血管直径和血流量 均大于 NMES 应用前(P<0.05),停用 NMES 后 1 h 这 3 个指标水平均恢复至 NMES 应用前。但 PFV 水平在应用 NMES 1.5 h 时出现波动下降,应用2 h时又回升稳定于应用1 h时的高峰水平。结论 NMES能加快重症患者下肢 静脉血液流动,应用NMES 1 h后达到高峰稳定期,建议临床应用NMES预防患者DVT时单次使用时长不短于1 h,增 加NMES的应用频次,两次间断时长不宜超过1 h。
Abstract:Objective To investigate the changes of hemodynamic indicators over time after neuromuscular electrical stimulation(NMES)in the prevention of the formation of deep venous thrombosis(DVT)in critically ill patients. Methods Forty patients in the intensive care unit(ICU)from the First Affiliated Hospital of Shandong First Medical University from June 2024 to August 2024 were selected as the research objects. Hemodynamic indicators of the popliteal vein[peak blood flow velocity(PFV),blood flow,vessel diameter]were collected by Doppler ultrasound before NMES application,30 min,1 h,1.5 h,2 h after NMES application,and 30 min,1 h after NMES discontinuation. Their temporal change patterns were analyzed. Results There were significant statistical differences in PFV,vessel diameter,and blood flow of popliteal vein across all time points in the 40 patients at different time points when NMSE was applied(χ2 =179.767,F=92.058,F=196.013,P<0.01). Comparison at different time points showed that PFV,vessel diameter,and blood flow at 30 min,1 h,1.5 h,and 2 h after NMES application were all higher than those before NMES application(P<0.05),and the three indicators returned to the baseline levels before NMES application 1 h after discontinuation. However,PFV fluctuated and decreased at 1.5 h after NMES application,and then rose again and stabilized at the peak level at 1 h after 2 h of application. Conclusion NMES can accelerate the venous blood flow in the lower limbs of critically ill patients,and the peak stable period is reached after 1 h of NMES application. It is recommended that the single use duration of NMES for DVT prevention in critically ill patients should not be less than 1 h,the application frequency should be increased,and the interval between two sessions should not exceed 1 h.
