中文摘要: 腮腺肿瘤是一种较为常见的唾液腺组织疾病,多数腮腺肿瘤属于上皮性肿瘤,少数来源于间叶组织(由中胚层的间充 质分化发育来的组织),临床主要表现为耳下前区肿块、面部疼痛、麻木、张口受限等,且不同类型的疾病具有不同的表现。腮 腺肿瘤的病理类型复杂,不同类型的腮腺肿瘤的手术方式不同,因此,准确诊断腮腺肿瘤的类型对其制定手术方案至关重 要。由于腮腺肿瘤软组织较多,组织学成分复杂、生物学行为差异较大,传统的影像学检查难以满足影像上定性的需求。随 着医疗数字化程度及医疗技术水平的提高,现代智能医疗逐渐将大数据与人工智能结合,有利于获取更深层次的影像数据信 息。影像组学是指从影像(超声、CT、MRI等)中高通量地提取大量影像信息,实现肿瘤分割、特征提取与模型建立,来辅助医 师做出最准确的诊断。影像组学基于医学图像的定量分析,能够量化肿瘤的异质性,对腮腺肿瘤的鉴别诊断具有重要的指导 意义。本文对影像组学在腮腺肿瘤鉴别诊断中的应用新进展进行综述,为临床准确诊断腮腺肿瘤提供资料参考。
Abstract:Parotid gland tumors are relatively common salivary gland disorders, with the majority being epithelial tumors and a minority originating from mesenchymal tissues (derived from mesodermal mesenchyme). Clinically, they primarily manifest as masses in the pre-auricular region, facial pain, numbness, and restricted mouth opening, with varying presentations depending on the type of tumor. The pathological types of parotid gland tumors are complex, and different types require different surgical approaches. Therefore, accurately diagnosing the type of parotid gland tumor is crucial for developing an appropriate surgical plan. Due to the abundance of soft tissue in parotid tumors, their complex histological composition, and significant variability in biological behavior, traditional imaging techniques often fall short in providing definitive qualitative assessments. With the advancement of medical digitization and technological improvements, modern intelligent healthcare has increasingly integrated big data and artificial intelligence, enabling the acquisition of deeper imaging data insights. Radiomics refers to the high-throughput extraction of large amounts of imaging information from modalities such as ultrasound, CT, and MRI, facilitating tumor segmentation, feature extraction, and model construction to assist physicians in making the most accurate diagnoses. Based on quantitative analysis of medical images, radiomics can quantify tumor heterogeneity and plays a significant role in the differential diagnosis of parotid gland tumors. This article reviews the latest advancements in the application of radiomics for the differential diagnosis of parotid gland tumors, providing valuable references for accurate clinical diagnosis.
