中文摘要: 高血压是全球范围内对人类健康造成严重威胁的一种常见慢性疾病,其心、脑、肾并发症给患者带来沉重的负担。研究发现,高血压的总体控制率尚未达到高血压人群的一半,难治性高血压(RH)因患者对多种抗高血压药物反应降低、临床表现复杂、病因特殊、预后和管理方面欠佳而更具有挑战性。阻塞性睡眠呼吸暂停低通气综合征(OSAHS)是RH发病的独立危险因素,其发病率呈现逐年递增的趋势,且患者未能得到及时的筛查和诊断,从而加速心血管疾病的过早发生。经皮肾动脉去交感神经消融术(RDN)可以精准破坏肾动脉壁内的交感神经纤维,有效降低交感神经的兴奋性,进而实现RH合并OSAHS患者血压的下降及通气功能的改善。本文旨在深入系统地剖析RH合并OSAHS的病理生理机制,并探讨RDN在该疾病治疗中的实际应用效果,以期为这一复杂的医学挑战提供坚实的理论基础与实践指导。

Abstract:Hypertension is a common chronic disease that poses a serious threat to human health all over the world. Its complications of heart,brain and kidney bring heavy burden to patients. The study found that the overall control rate of hypertension is less than half that of people with hypertension,and patients with refractory hypertension(RH)are more challenging due to their reduced responses to multiple antihypertensive drugs,complex clinical manifestations,special etiology,and poor prognosis and management. Obstructive sleep apnea-hypopnea syndrome(OSAHS)is an independent risk factor for RH. Its prevalence increases year by year and patients are not screened and diagnosed in time,which leads to premature occurrence of cardiovascular diseases. Renal denervation(RDN)can specifically destroy the sympathetic nerve fibers in the renal artery wall. It effectively reduce the excitability of the sympathetic nerve,reduce blood pressure,and improve ventilation in patients with RH complicated with OSAHS. This article aims to comprehensively and deeply explore the pathogenesis of RH complicated with OSAHS and the application effects of RDN in the treatment of this disease,so as to provide new theoretical support and practical solutions for solving this complicated medical problem.
