中文摘要: 目的 探讨三维动脉自旋标记(3D-ASL)脑灌注成像技术联合高清弥散成像技术在急性缺血性脑卒中患者的应用价值,为疾病的早期诊断提供参考。方法 选取2022年10月至2023年12月于中山市小榄人民医院收治的疑似急性缺血性脑卒中患者80例作为观察组,另选取同期70例健康受检者作为对照组,两组均进行3D-ASL 脑灌注成像、高清弥散成像检查,测量对照组及观察组左侧大脑中动脉供血区与右侧相应区域脑血流量(CBF)绝对值。采用受试者工作特征(ROC)曲线分析3D-ASL脑灌注成像技术、高清弥散成像技术及联合诊断对急性缺血性脑卒中的诊断效能。结果 与对照组相比,观察组CBF绝对值、左侧CBF绝对值低(P<0.05);在左侧高信号中,与左侧CBF绝对值(19.07±2.19)相比,右侧CBF绝对值(41.38±4.82)高(t=20.210,P <0.01);在右侧高信号中,与左侧CBF绝对值(33.47±2.95)相比,右侧CBF绝对值(16.08±1.49)低(t=32.436,P <0.01)。3D-ASL脑灌注成像技术联合高清弥散成像技术诊断急性缺血性脑卒中的ROC 曲线下面积为0.801(95%CI:0.663~0.938),敏感度为93.44%,特异度为63.16%,准确度为86.25%。结论 急性缺血性脑卒中患者行3D-ASL脑灌注成像技术联合高清弥散成像技术具有较高的诊断效能,且可有效观察脑血流灌注情况。

Abstract:Objective To explore the application value of three -dimensional arterial spin labeling(3D -ASL)cerebral perfusion imaging combined with high - resolution diffusion imaging in patients with acute ischemic stroke,and to provide a reference for the early diagnosis of the disease. Methods Eighty patients suspected of acute ischemic stroke who were admitted to Xiaolan People’'s Hospital of Zhongshan from October 2022 to December 2023 were selected as the observation group,and another 70 healthy subjects during the same period were selected as the control group. Both groups underwent 3D-ASL cerebral perfusion imaging and high-resolution diffusion imaging examinations. The absolute values of cerebral blood flow(CBF)in the blood supply area of the left middle cerebral artery and the corresponding right area were measured in both the control group and the observation group. The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of 3D - ASL cerebral perfusion imaging technology,high -resolution diffusion imaging technology,and their combined diagnosis for acute ischemic stroke. Results Compared with the control group,the absolute values of CBF and left CBF in the observation group were lower(P<0.05). In the high signal on the left,compared with the absolute value of CBF on the left(19.07±2.19),the absolute value of CBF on the right(41.38±4.82)was higher(t=20.210,P <0.01);In the high signal on the right,compared with the absolute value of CBF on the left(33.47±2.95),the absolute value of CBF on the right(16.08±1.49)was lower(t=32.436,P<0.05);The area under the ROC curve(AUC)of 3D-ASL cerebral perfusion imaging technology combined with high-resolution diffusion imaging technology for diagnosing acute ischemic stroke was 0.801(95%CI:0.663-0.938),with a sensitivity of 93.44%,a specificity of 63.16%,and an accuracy of 86.25%. Conclusion The application of 3D-ASL cerebral perfusion imaging combined with high - resolution diffusion imaging in patients with acute ischemic stroke has high diagnostic efficacy and can effectively observe the status of cerebral blood perfusion.
