中文摘要: 目的 研究糖化血清蛋白(GSP)、糖化血红蛋白(HbA1C)及其交互作用,并分析其与妊娠期糖尿病孕妇胰岛素抵抗(IR)的关系。方法 选取2020年2月至2024年2月于新疆四七四医院进行治疗的110例妊娠期糖尿病孕妇进行回顾性分析,根据是否伴有IR将孕妇进行分为IR组(n=32)和非IR组(n=78)。对比两组的临床资料及GSP、HbA1C水平,采用多因素logistic回归分析妊娠期糖尿病孕妇发生IR的影响因素,并分析GSP、HbA1C及其交互作用与妊娠期糖尿病孕妇IR的关联。采用多元线性回归分析GSP、HbA1C与稳态模型胰岛素抵抗指数(HOMA-IR)的相关性。采用受试者工作特征(ROC)曲线评估GSP、HbA1C及其交互作用对妊娠期糖尿病孕妇发生IR的预测价值。结果 IR组和非IR组孕妇的妊娠期身体质量指数(BMI)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、GSP、HbA1C、空腹血糖(FPG)、空腹胰岛素(FINS)、HOMA-IR等比较差异有统计学意义(P<0.05)。多因素logistic回归分析显示,妊娠期BMI、GSP、HbA1C、FPG、FINS、HOMA-IR等指标升高均为影响妊娠期糖尿病孕妇发生IR的独立危险因素(P<0.05)。多元线性回归分析结果显示,GSP、HbA1C均与HOMA-IR呈正相关(P<0.05)。相乘交互模型显示,GSP、HbA1C对妊娠期糖尿病孕妇发生IR无相乘交互作用( P> 0.05)。相加交互模型显示,GSP、HbA1C对妊娠期糖尿病孕妇发生IR有相加交互作用(P<0.05);GSP、HbA1C高水平的妊娠期糖尿病孕妇发生IR的概率显著高于存在GSP、HbA1C低水平的妊娠期糖尿病孕妇(P<0.05)。GSP的ROC曲线下面积(AUC)为0.831,HbA1C的AUC为0.805,二者相加交互作用的AUC为0.882(P<0.05)。结论 GSP、HbA1C对妊娠期糖尿病孕妇发生IR具有相加交互作用,且GSP、HbA1C及其交互作用预测IR的临床价值较高。
Abstract:Objective To study glycated serum protein, glycated hemoglobin HbA1C and their interactions, and to analyze their relationship with insulin resistance (IR) in pregnant women with gestational diabetes mellitus. Methods A total of 110 pregnant women with gestational diabetes mellitus who were treated in the Xinjiang 474 Hospital from February 2020 to February 2024 were selected. The pregnant women were divided into IR group (n=32) and non-IR group (n=78) according to whether they were complicated with IR. The clinical data, GSP and HbA1C levels of the two groups were compared. Multivariate logistic regression was used to analyze the influencing factors of IR in pregnant women with gestational diabetes mellitus, and the association of GSP, HbA1C and their interaction with IR in pregnant women with gestational diabetes mellitus was analyzed. Multiple linear regression was used to analyze the correlation between GSP, HbA1C and homeostasis model assessment of insulin resistance (HOMA-IR). Receiver operating curve (ROC) curve was used to evaluate the GSP, HbA1C and their interactions on IR in pregnant women with gestational diabetes mellitus. Results Significant differences in body mass index (BMI) during pregnancy, triglycerides (TG), high-density lipoprotein cholesterol (HDL-C), GSP, HbA1C, fasting plasma glucose (FPG), fasting insulin (FINS), and HOMA-IR were observed between the IR and non-IR groups (P<0.05). Multinominal logistic regression analysis revealed that increased BMI during pregnancy, GSP, HbA1C, FPG, FINS, and HOMA-IR were independent risk factors for the occurrence of IR in pregnant women with gestational diabetes mellitus (P<0.05). The results of multiple linear regression analysis indicated that both GSP and HbA1C were positively correlated with HOMA- IR (P<0.05). The multiplicative interaction model showed that there was no multiplicative interaction between GSP and HbA1C for the occurrence of IR in pregnant women with gestational diabetes mellitus ( P> 0.05). The additive interaction model demonstrated that GSP and HbA1C had an additive interaction on the occurrence of IR in pregnant women with gestational diabetes mellitus (P<0.05) ; the probability of IR in pregnant women with gestational diabetes mellitus who had high expression of GSP and HbA1C was significantly higher than in those with low expression of GSP and HbA1C (P<0.05). The ROC curve showed that the area under the curve (AUC) for GSP was 0.831, for HbA1C was 0.805, and for their additive interaction was 0.882 (P<0.05). Conclusion GSP and HbA1C have an additive interaction effect on the occurrence of IR in pregnant women with gestational diabetes mellitus, and GSP, HbA1C and their interaction have high clinical value in predicting IR.
