中文摘要: 目的 探讨强化他汀类药物干预对脑动脉硬化症(CAS)患者血脂水平改善与斑块稳定性的影响效应,为临床治疗提供参考。方法 选择2023年1月至2024年5月在信阳市人民医院接受治疗的CAS患者103例,以随机数表法分为研究组(n=52)和基础组51例(n=51)。在基础治疗后,基础组联合小剂量阿托伐他汀钙(10 mg/d)治疗,研究组联合大剂量阿托伐他汀钙(40 mg/d,持续使用14 d后改为20 mg/d)治疗,均持续治疗6个月。对比两组患者治疗前后血脂水平[总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]以及斑块情况[颈动脉内-中膜厚度(IMT)、颈动脉斑块面积]、血流动力学指标[血管阻力指数(RI)、血管搏动指数(PI)、收缩期血流速度(Vs)、舒张期血流速度(Vd)]、炎症因子水平[肿瘤坏死因子-α(TNF-α)、高敏C 反应蛋白(hs-CRP)、白细胞介素-1(IL-1)]等。结果 治疗后两组TC、TG、HDL-C、LDL-C、RI、PI、Vs、Vd、TNF-α、hs-CRP 及IL-1较治疗前均改善,且研究组优于基础组(P<0.05)。治疗后两组颈动脉斑块面积均较治疗前稍有降低,但差异无统计学意义(P>0.05);研究组治疗后IMT 低于治疗前(t=5.294,P<0.001)以及基础组(t=2.521,P=0.013)。结论 采用大剂量阿托伐他汀钙治疗CAS,不仅能改善血脂水平,有利于降低颈动脉IMT,且可调节血流动力学指标,减少炎症反应。对强化剂量下的不良反应有待进一步观察。
Abstract:Objective To investigate the effects of intensive statin intervention on lipid profile improvement and plaque stability in patients with cerebral atherosclerosis(CAS),and to provide references for clinical treatment. Methods A total of 103 patients with CAS treated at Xinyang People's Hospital from January 2023 to May 2024 were selected and randomly divided into a study group(n=52)and a basic group(n=51)using a random number table method. After basic treatment,the basic group received low-dose atorvastatin calcium(10 mg/d),while the study group received high-dose atorvastatin calcium(40 mg/d for 14 days followed by 20 mg/d thereafter),all of which continued for 6 months. The two groups were compared before and after treatment in terms of lipid levels[total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C)],plaque parameters[carotid intima - media thickness(IMT),carotid plaque area],hemodynamic parameters[resistance index(RI),pulsatility index(PI),systolic flow velocity(Vs),diastolic flow velocity(Vd)],and inflammatory factor levels[tumor necrosis factor-alpha(TNF-α),high-sensitivity C-reactive protein(hs-CRP),interleukin-1(IL-1)]. Results After treatment,TC,TG,HDL-C,LDL-C,RI,PI,Vs,Vd,TNF-α,hs-CRP,and IL-1 improved in both groups compared with before treatment,and the magnitude of change was greater in the study group than in the basic group(P<0.05). After treatment,the carotid plaque area in both groups decreased slightly compared to before treatment,with no statistically significant differences(P>0.05). The study group showed lower post-treatment IMT than pre-treatment(t=5.294,P<0.001)and the basic group(t=2.521, P=0.013). Conclusion High - dose atorvastatin calcium treatment for CAS can not only improve lipid profiles,reduce the IMT,but also regulate hemodynamic parameters and reduce inflammatory responses.The adverse reactions to intensive doses need further observation.
