中文摘要: 目的  探讨二胺氧化酶、D-乳酸、细菌内毒素等肠道屏障功能生化指标在急性胰腺炎严重程度中的预测价值。方法  回顾性收集2021年12月至2023年1月于南京江北医院收治的139例急性胰腺炎患者,将其分为轻症组(轻症急性胰腺炎,n=39)和非轻症组(中度重症急性胰腺炎及重症急性胰腺炎,n=100),收集并比较两组患者的血清二胺氧化酶、D-乳酸、细菌内毒素以及急性生理学与慢性健康状况评价Ⅱ(APACHE Ⅱ)评分、急性胰腺炎床旁严重程度指数(BISAP)评分、改良CT严重指数(MCTSI)评分,采用Spearman相关性分析探讨各指标间的相关性,采用受试者工作特征(ROC)曲线分析各指标对非轻症胰腺炎的预测价值。结果  非轻症组二胺氧化酶、D-乳酸、细菌内毒素、APACHE Ⅱ评分、BISAP 评分、MCTSI 评分均高于轻症组,血钙水平低于轻症组(P<0.05)。Spearman相关性分析结果显示,两组二胺氧化酶、D-乳酸、细菌内毒素水平与APACHE Ⅱ评分、BISAP评分、MCTSI评分分别呈正相关(P<0.05)。ROC曲线结果显示,二胺氧化酶、D-乳酸、细菌内毒素预测非轻症急性胰腺炎的曲线下面积(AUC)分别为0.690、0.749、0.718;且以D-乳酸联合BISAP评分预测的AUC最大,为0.852。结论  肠道屏障功能生化指标在预测急性胰腺炎严重程度中具有一定的预测价值,且联合评分系统预测效能更高。
Abstract:Objective To investigate the predictive value of biochemical indicators of intestinal barrier function,such as diamine oxidase,D-lactate and bacterial endotoxin for the severity of acute pancreatitis. Methods A total of 139 patients with acute pancreatitis admitted to Nanjing Jiangbei Hospital from December 2021 to January 2023 were retrospectively enrolled. The patients were divided into mild group(mild acute pancreatitis,n=39)and non-mild group(moderately severe acute pancreatitis and severe acute pancreatitis,n=100). The serum levels of diamine oxidase,D-lactate,and bacterial endotoxin,as well as Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score,Bedside Index of Severity in Acute Pancreatitis(BISAP)score and Modified Computed Tomography Severity Index(MCTSI)score were collected and compared between the two groups. Spearman correlation analysis was used to analyze the correlation among the above indicators,and receiver operating characteristic(ROC)curve was adopted to evaluate the predictive value of each indicator for non-mild acute pancreatitis. Results Compared with mild group,the levels of diamine oxidase,D-lactate,bacterial endotoxin,APACHE Ⅱscore,BISAP score and MCTSI score in the non-mild group were significantly higher,while the blood calcium level was lower(P<0.05). In both groups,spearman correlation analysis showed that the levels of diamine oxidase,D - lactate and bacterial endotoxin were positively correlated with APACHE Ⅱ,BISAP and MCTSI scores,respectively(P<0.05). ROC curve analysis revealed that the area under the curve(AUC)of diamine oxidase,D - lactate and bacterial endotoxin for predicting non - mild acute pancreatitis was 0.690,0.749 and 0.718,respectively,and the combination of D-lactate and the BISAP score presented the largest AUC of 0.852. Conclusion Biochemical indicators of intestinal barrier function have certain predictive value for evaluating the severity of acute pancreatitis,and their combination with scoring systems can achieve higher predictive efficacy.
