中文摘要: 肩部手术,包括肩关节镜、锁骨骨折切开复位内固定、肱骨近端骨折切开复位内固定,是十分常见的骨科手术类型。由于术中电切灼烧、持续关节腔冲洗等,术后常伴随剧烈疼痛。超声可视下的臂丛神经阻滞,不仅能减轻手术应激反应,为患者提供术后镇痛,有利于早期功能锻炼,还可提供充分的外科手术麻醉,避免全麻气管插管,为高危患者极大降低麻醉手术风险。但臂丛神经阻滞最常见的副作用,即半侧膈肌麻痹(HDP),限制了其在病态肥胖、合并神经肌肉疾病等已存在呼吸功能不全患者中的使用,但合并呼吸功能不全的患者往往最能从神经阻滞中获益。基于此,许多关于保留膈肌功能的臂丛阻滞研究相继开展。本综述总结了此类阻滞技术的进展,为其临床实际应用提供参考。
Abstract:Shoulder surgeries,including shoulder arthroscopy,open reduction and internal fixation(ORIF)for clavicular fractures and proximal humeral fractures,are common orthopedic procedures. Severe postoperative pain frequently occurs following these surgeries due to intraoperative electrocautery,continuous intra-articular irrigation and other operative factors. Ultrasound-guided brachial plexus block can effectively reduce surgical stress responses,provide satisfactory postoperative analgesia and facilitate early functional rehabilitation. It can also achieve sufficient surgical anesthesia and avoid tracheal intubation under general anesthesia,thereby significantly reducing perioperative anesthetic risks for high-risk patients. However,hemidiaphragmatic paralysis(HDP),the most prevalent adverse effect of brachial plexus block,limits its clinical application in patients with preexisting respiratory insufficiency,such as those with morbid obesity and neuromuscular diseases. Notably,patients with respiratory dysfunction are often the population who benefit the most from nerve block anesthesia. Accordingly,numerous studies have focused on brachial plexus block techniques with diaphragmatic function preservation. This review summarizes the research progress of these modified block techniques,so as to provide a reference for their clinical application.
