中文摘要: 目的 探讨通过肌骨超声测量腋隐窝滑囊厚度(APT)在冻结肩诊断及评估中的应用价值。方法 回顾性分析2023年1月至2025年1月于江苏省中医院超声医学科就诊的冻结肩患者168例,采用肌骨超声成像测量患者的APT,并使用肩关节疼痛与功能障碍指数(SPADI)评估患者肩关节功能状况,分析APT在冻结肩不同分期及严重程度患者中的差异。结果 共168例冻结肩患者行肌骨超声检查。按病程分为急性期组(≤3个月,64例)、慢性期组(4~6个月,63例)和恢复期组(>6个月,41例),APT分别为(4.04±0.73)mm、(3.97±0.93)mm和(3.40±0.63)mm,SPADI分别为50.38±17.10、49.60±19.91和42.61±19.95,三组APT比较差异均有统计学意义(F=9.213,P<0.01),三组SPADI比较差异均无统计学意义(P>0.05)。按SPADI分为轻度障碍组(0~30分,33例)、中度障碍组(31~60分,89例)、重度障碍组(61~80分,36例)和极重度障碍组(81~100分,10例)。轻度障碍组、中度障碍组、重度障碍组和极重度障碍组患者的APT分别为(3.43±0.89)mm、(4.02±0.81)mm、(3.84±0.63)mm和(3.93±1.00)mm,四组APT差异有统计学意义(F=4.356,P=0.006),且以轻度障碍组患者的APT最小(P<0.05)。结论 肌骨超声成像是一种快速、价廉且非侵入性的技术,APT在冻结肩急性期最高,在轻度肩关节功能障碍患者中最低,可以协助临床对冻结肩进行诊断。

Abstract:Objective To explore the application value of measuring axillary pouch thickness(APT)by musculoskeletal ultrasound in the diagnosis and evaluation of frozen shoulder. Methods A total of 168 patients with frozen shoulder who visited the Department of Ultrasound Medicine of Jiangsu Provincial Hospital of Chinese Medicine from January 2023 to January 2025 were selected. The APT was measured by musculoskeletal ultrasound imaging, and the Shoulder Pain and Disability Index(SPADI)was used to assess shoulder function. The differences of APT and SPADI scores in patients with frozen shoulder in different stages and severity were analyzed. Results A total of 168 frozen shoulder patients underwent musculoskeletal ultrasound examination. Patients were divided into three groups according to disease course:acute phase group(≤3 months, n=64), chronic phase group(4-6 months, n=63), and recovery phase group(>6months, n=41).The APTs were(4.04±0.73)mm, (3.97±0.93)mm, and(3.40±0.63)mm, respectively, and the SPADI scores were 50.38±17.10, 49.60±19.91 and 42.61±19.95, respectively. There were statistically significant differences in APT among the three groups(F=9.213, P<0.01), but the difference in SPADI among the three groups was not statistically significant(P>0.05). Based on SPADI scores, the patients were categorized into four groups:mild disability group(0-30 points, 33 cases), moderate disability group(31-60 points, 89 cases), severe disability group(61-80points, 36 cases), and very severe disability group(81-100 points, 10 cases). The APTs of patients in the mild disability group, moderate disability group, severe disability group, and very severe disability group were(3.43±0.89)mm, (4.02±0.81)mm, (3.84±0.63)mm, and(3.93±1.00)mm, respectively, and the differences among four groups were statistically significant(F=4.356, P=0.006), with the mild disability group having the smallest APT(P<0.05).Conclusion Musculoskeletal ultrasound is a rapid, inexpensive and non-invasive technique that can assist in the clinical diagnosis of frozen shoulder. APT is highest in the acute phase of frozen shoulder and lowest in patients with mild shoulder pain and disability, which can assist clinicians in diagnosing frozen shoulder.
