中文摘要: 目的 探讨矮小同源盒基因2(SHOX2)与分隔蛋白9(SEPT9 )基因甲基化在不同程度宫颈病变中的表达,及二者在宫颈上皮内瘤变(CIN)3级和宫颈鳞状细胞癌(CSCC)诊断中的应用价值。方法 回顾性分析2021年6月至2022年5月在南通大学附属医院妇科门诊就诊并行宫颈活检的112例患者,依据组织病理结果分为慢性宫颈炎组(n=17)、CIN Ⅰ组(n=24)、CIN Ⅱ组(n=22)、CIN Ⅲ组(n=27)和 CSCC 组(n=22)。提取宫颈标本DNA,经亚硫酸氢盐转化后进行 SHOX2 和 SEPT9 甲基化特异性聚合酶链式反应检测,计算ΔCt 值评估甲基化水平。采用受试者工作特征(ROC)曲线和曲线下面积(AUC)评估 SHOX2、SEPT9 及其联合甲基化在不同宫颈病变中的诊断效能。结果 SHOX2与SEPT9甲基化ΔCt水平随病变程度加重而升高,五组间阳性检出率差异有统计学意义(P<0.01)。SHOX2甲基化在CIN Ⅲ与CSCC中的阳性率分别为62.96%和90.91%,SEPT9分别为70.37%和95.45%,联合检测分别达77.78%和100.00%。ROC曲线显示,SHOX2、SEPT9甲基化及二者联合甲基化诊断CIN Ⅲ的AUC分别为0.662、0.681和0.700,SHOX2、SEPT9甲基化及二者联合甲基化诊断CSCC的AUC分别为0.827、0.827和0.870。结论 SHOX2和SEPT9甲基化水平与宫颈病变程度密切相关。两者联合检测在CIN Ⅲ和CSCC的识别中表现出较高的诊断效能。
Abstract:Objective To investigate the expression of Short stature homeobox 2(SHOX2)and Septin 9(SEPT9)genemethylation in cervical lesions of varying severity,and to evaluate their application value in the diagnosis of cervical intraepithelial neoplasia grade 3(CIN Ⅲ)and cervical squamous cell carcinoma(CSCC). Methods A retrospective analysis was performed on 112 patients who underwent cervical biopsy at the Gynecology Clinic of Affiliated Hospital of Nantong University from June 2021 to May 2022. According to histopathological results,the patients were divided into chronic cervicitis group(n=17),CIN Ⅰ group(n=24),CIN Ⅱ group(n=22),CIN Ⅲ group(n=27)and CSCC group(n=22). DNA was extracted from cervical specimens,and after bisulfite conversion,methylation - specific polymerasechain reaction was performed for SHOX2 and SEPT9. The ΔCt value was calculated to assess the methylation level.Receiver operating characteristic(ROC)curves and area under the curve(AUC)were used to evaluate the diagnostic efficacy of SHOX2 methylation,SEPT9 methylation and their combined methylation in different cervical lesions.Results The methylation levels of SHOX2 and SEPT9 increased with the aggravation of lesion severity,and there were statistically significant differences in positive detection rates among the five groups(P<0.01). The positive rates ofSHOX2 methylation in CIN Ⅲ and CSCC were 62.96% and 90.91%,respectively,while those of SEPT9 methylationwere 70.37% and 95.45%,respectively,and the combined detection reached 77.78% and 100.00%,respectively. ROC curves showed that the AUCs of SHOX2 methylation,SEPT9 methylation and their combined methylation for the diagnosis of CIN Ⅲ were 0.662,0.681 and 0.700,respectively,and the AUCs for the diagnosis of CSCC were 0.827,0.827 and0.870,respectively. Conclusion The methylation levels of SHOX2 and SEPT9 are closely related to the severity ofcervical lesions. Combined detection of the two genes exhibits high diagnostic efficacy in identifying CIN Ⅲ and CSCC.
