中文摘要: 目的 评价5A模式在困难性下腔静脉滤器（IVCF）取出术患者围术期的应用及其效果。方法 选取2020年6月至2022年6月在北京积水潭医院治疗的困难性IVCF取出术患者共140例为研究对象，根据护理方法分为5A模式组（n=66）及常规护理组（n=74）。5A模式组采用5A模式护理，常规护理组采用常规护理。比较两组患者的围术期并发症发生率、自我护理能力、负性情绪、护理满意度、远期并发症发生率。结果 护理实施后，5A模式组患者围术期并发症发生率低于对照组（18.18% vs 35.14%， P＜0.05）。5A模式组自我护理能力各项评分均高于常规护理组（P＜0.05）。实施护理后，两组HAMA、HAMD评分均低于护理前，且5A模式组汉密乐顿焦虑量表（HAMA）、汉密尔顿抑郁量表（HAMD）评分均低于常规护理组（P＜0.05）。5A模式组护理满意度高于常规护理组（92.42% vs 79.73%, P＜0.05）。滤器取出术6个月后，5A模式组患者远期并发症总发生率低于常规护理组（12.12% vs 18.92%, P＜0.05）。结论 5A模式护理应用于困难性IVCF取出术患者有利于提升患者自护能力，缓解患者负性情绪，降低并发症发生率，提升患者护理满意度。
Abstract:Objective To evaluate the application effect of 5A mode in perioperative period of patients undergoing difficult inferior vena cava filter (IVCF) retrieval. Methods A total of 140 patients undergoing difficult IVCF retrieval in the Beijing Jishuitan Hospital were enrolled between June 2020 and June 2022. According to different nursing methods, they were divided into 5A mode group (66 cases, 5A mode nursing) and routine nursing group (74 cases, routine nursing). The incidence of perioperative complications, self-care ability, negative emotions,clinical nursing satisfaction and incidence of long-term complications in the two groups were compared. Results After nursing, incidence of perioperative complications in 5A mode group was lower than that in routine nursing group (18.18% vs 35.14%, P＜0.05). Scores of self-care ability in 5A mode group were higher than in routine nursing group (P＜0.05). HAMA and HAMD scores were decreased in both groups, which were lower in 5A mode group than routine nursing group (P＜0.05). The nursing satisfaction rate in 5A mode group was higher than that in routine nursing group (92.42% vs 79.73%, P＜0.05), and incidence of long-term complications was lower than that in routine nursing group (12.12% vs 18.92%, P＜0.05). Conclusion 5A mode is beneficial to improve self-care ability, alleviate negative emotions, reduce the incidence of complications and improve nursing satisfaction in patients undergoing difficult IVCF retrieval.
