中文摘要: 目的 探讨应用翻转静脉导管联合神经断端平移修复周围神经缺损的可行性。 方法 将36只SD大鼠按手术方法随机分为3组,A组受损神经不做任何处理;B组用翻转静脉导管连接神经缺损断端;C组用翻转静脉导管联合神经断端平移修复周围神经缺损。术后定期观察并记录各组动物左下肢创面恢复情况及肢体活动情况。术后3、5、7和13周进行电生理检查和组织病理学观察。 结果 术后一般情况观察,C组大鼠肢体活动恢复较A组、B组快;电生理检测仪检测各组大鼠左下肢坐骨神经缺损修复后的神经传导速度(NCV)和神经复合动作电位(CMAP)振幅(AMP)在术后3周时差异无统计学意义(P>0.05)。从术后5周开始,C组NCV和CMAP振幅均优于A组、B组,差异有统计学意义(P<0.05)。C组术后7周、13周M1和M2处有髓神经纤维计数、髓鞘厚度、轴突直径优于A组、B组,差异有统计学意义(P<0.05)。 结论 应用翻转静脉导管联合神经断端平移修复周围神经大段缺损的效果优于单纯翻转静脉,该方法可作为神经损伤修复的一种选择。
Abstract:Objective To explore the feasibility of repairing peripheral nerve defects by using inverted venous catheter combined with translation of nerve stump. Methods Thirty-six SD rats were randomly divided into three groups according to the surgical method. No procedures was performed in group A, the broken end of nerve defect was connected with the inverted venous catheter in group B, and the peripheral nerve defect was repaired with the inverted venous catheter combined with translation of nerve stump in group C. The wound recovery and limb movement of animals were observed and recorded among three groups after operation. Electrophysiological examinations and histopathological observations were performed at 3-, 5-, 7- and 13- week after operation. Results The recovery of limb activity in group C was faster than those in groups A and B. There were no significant differences in nerve conduction velocity(NCV) and amplitude of compound muscle action potential(CMAP) amplitude at 3 weeks after repairing sciatic nerve defect in the left lower limb of rats among three groups(P>0.05). From 5 weeks after operation, NCV and amplitude of CMAP in group C were significantly better than those in group A and B(P<0.05).The number of myeliuated nerve fibers, myelin sheath thickness and axon diameter at M1 and M2 in group C were superior to those in group A and B at 7 and 13 weeks after operation(P<0.05). Conclusion In repairing large segmental defects of peripheral nerves, the effect of inverted venous catheter combined with translation of nerve stump is better than that of simple inverted vein and and can be used as a choice for nerve injury repair.
