中文摘要: 目的调查和分析某三甲医院静脉血栓栓塞症（VTE）管理前后的防治状况及应用效果，以完善住院患者VTE的规范化防治管理。
方法以2021年5月3日至31日淮安市第一人民医院部分科室住院患者为研究组，以实行院内VTE管理前2016—2018年（每年5月3日至31日）住院患者为对照组，查阅电子病历系统，收集患者一般情况、Padua和Caprini风险评分、预防措施、治疗方案等详细临床资料，进行分析和讨论。
结果2021年VTE的诊断率较院内VTE管理实施前的2016—2018年显著提高（P＜0.05），住院期间VTE诊断率显著上升（P＜0.05），VTE病死率显著下降（P<0.01）;VTE风险动态评估与VTE发生显著相关（P＜0.01）。
结论目前实施的院内VTE管理体系，有助于提高院内VTE的诊断，使住院期间病死率下降，但仍需完善。
Abstract:ObjectiveToinvestigatethepreventionandtreatmentstatusbeforeandaftermanagementofvenousthromboembolism（VTE）inatertiaryhospital,soastostandardizethepreventionandtreatmentstrategyforthehospitalizedpatients.
MethodsThepatientshospitalizedinHuaianFirstPeoplesHospitalfromMay3to31,2021wereselectedasresearchgroup（aftermanagementofVTE）,andtheinpatientsfrom2016to2018（May3to31ineachyear，beforeimplementationofVTE）wereservedascontrolgroup.Throughtheelectronicmedicalrecord（EMR）system,thepatientsgeneralcondition,PaduaandCapriniriskscores,preventivemeasuresandtreatmentplanswerecollectedforanalysisanddiscussion.
ResultsThediagnosticrateofVTEin2021wassignificantlyhigherthanthatin2016-2018beforein-hospitalVTEmanagement（P＜0.05）.Duringhospitalization,thediagnosisrateofVTEincreasedsignificantly(P<0.05),andthemortalityrateofVTEdecreasedsignificantly(P<0.01).VTEriskdynamicassessmentwassignificantlyassociatedwiththeoccurrenceofVTE（P＜0.01）.
ConclusionThecurrentlyimplementedin-hospitalVTEmanagementsystemishelpfultoimprovethediagnosisofin-hospitalVTE,resultinginalowmortalityduringhospitalization,butitstillneedstobeimproved.
