中文摘要: 目的探讨红细胞分布宽度（RDW）在原发性高血压（EH）人群中的分布特点及其与EH严重程度的关系，评价RDW对EH的诊断价值。
方法选择2019年7月至2021年7月在扬州大学附属医院心血管内科住院初诊的EH患者300例，对照组为同期健康体检者105例，同时依据指南将EH患者分为高血压1级组、高血压2级组和高血压3级组，各100例。比较两组及EH各亚组间的RDW水平，分析RDW与EH的相关性及EH的独立危险因素。
结果EH组RDW水平显著高于对照组（P＜0.01），RDW在EH各亚组水平均高于正常对照组（P＜0.05）。多因素logistic回归分析显示，除三酰甘油、低密度脂蛋白胆固醇、纤维蛋白原、吸烟、年龄等因素外，RDW（OR=5.786，95%CI：2.998～11.165，P＜0.01）也是EH的独立危险因素。ROC曲线分析显示，RDW诊断EH的界点值为11.95%（特异度为58%，敏感度为83%）。
结论RDW与EH及其严重程度有关，是EH的独立危险因素，RDW可为EH的预测提供一定依据。
Abstract:ObjectiveToinvestigatethedistributioncharacteristicsofredbloodcelldistributionwidth（RDW）inthepopulationwithessentialhypertension（EH）anditsrelationshipwiththeseverityofEHtoevaluatethediagnosticvalueofRDWforEH.
MethodsAtotalof300patientswithnewlydiagnosedEHhospitalizedintheAffiliatedHospitalofYangzhouUniversityfromJuly2019toJuly2021wereselectedassubjects（EHgroup）,and105ofhealthyphysicalexaminationpersonnelinthesameperiodwereincludedinthecontrolgroup.Accordingtothediagnosticcriteriaofhypertension,EHpatientsweredividedintohypertensiongrade1group,hypertensiongrade2groupandhypertensiongrade3group（n=100,each）.TheRDWlevelswerecomparedbetweentwogroupsandamongthesubgroupstoanalyzethecorrelationbetweenRDWandEHandtheindependentriskfactorsforEH.
ResultsThelevelofRDWinEHgroup（includingthreesubgroups）wassignificantlyhigherthanthatincontrolgroup（P＜0.05）.MultivariatelogisticregressionanalysisshowedthatRDW（OR=5.786，95%CI：2.998-11.165，P＜0.01）wasalsoanindependentriskfactorforEHinadditiontotriacylglycerol,lowdensitylipoproteincholesterol,fibrinogen,smoking,ageandotherfactors.TheROCcurveanalysisshowedthatthecut-offvalueofRDWforEHwas11.95%,withaspecificityof58%andsensitivityof83%.
ConclusionRDWiscorrelatedtoEHanditsseverity,andistheindependentriskfactorforEH,whichcanprovidecertainbasisforthepredicitionofEH.
