CDX-2、PD-L1、Ki-67在大肠癌组织中的表达及与临床病理、预后的关系

中文摘要: 目的 观察大肠癌组织中尾型同源盒转录因子-2(CDX-2)、程序性死亡配体1(PD-L1)、细胞增殖核抗原(Ki-67)表达情况,并分析其与临床病理、预后的关系。方法 选取2016年1月至2018年1月江汉大学附属医院267例大肠癌患者手术切除标本及对应的癌旁组织(距离大肠癌病灶5 cm以上),采用免疫组织化学染色检测癌组织及癌旁正常组织中CDX-2、PD-L1、Ki-67表达情况,并分析其表达与大肠癌患者临床病理特征及预后的关系。结果 CDX-2在大肠癌癌组织中的阳性表达率显著低于癌旁正常组织(P<0.01),PD-L1、Ki-67阳性表达率显著高于对照组(P<0.01)。Kaplan-Meier生存分析显示,CDX-2阳性表达组总生存率显著高于CDX-2阴性表达组(68.00% vs 48.91%,P<0.01),PD-L1阳性表达组总生存率显著低于PD-L1阴性表达组(45.24% vs 88.89%,P<0.01),Ki-67阳性表达组总生存率显著低于Ki-67阴性表达组(58.44% vs 91.67%,P<0.01)。多因素回归分析显示,TNM分期Ⅲ期和Ⅳ期、淋巴结转移、CDX-2阴性表达、PD-L1阳性表达、Ki-67阳性表达是大肠癌患者预后的独立危险因素(P<0.05)。结论 CDX-2在大肠癌组织中表达显著下调,PD-L1、Ki-67在大肠癌组织中表达显著上调,CDX-2、PD-L1、Ki-67异常表达可能参与大肠癌发生发展,并与大肠癌预后密切相关。
Abstract:ObjectiveTo observe the expressions of caudal type homeobox transcription factor 2 (CDX-2), programmed death ligand 1 (PD-L1) and proliferating cell nuclear antigen (Ki-67) in colorectal cancer tissues and their relationshiPwith clinicopathology and prognosis. Methods The surgical resection specimens of 267 patients with colorectal cancer admitted to the hospital between January 2016 and January 2018 were collected as study group, and the adjacent normal tissues of corresponding patients (more than 5 cm from the colorectal cancer lesions) were used as control group. The expressions of CDX-2, PD-L1 and Ki-67 in cancer tissues and adjacent normal tissues were detected by immunohistochemical staining, and the relationshiPbetween their expressions and clinicopathological characteristics and prognosis of patients with colorectal cancer was analyzed. Results The positive expression rate of CDX-2 in colorectal cancer tissues was significantly lower than that in adjacent normal tissues (P<0.01), while the positive expression rates of PD-L1 and Ki-67 were significantly higher than those in control grouP(P<0.01). Kaplan-Meier survival analysis showed that the overall survival rate of CDX-2 positive expression grouPwas significantly higher than that of CDX-2 negative expression grouP(68.00% vs 48.91%,P<0.01), and the overall survival rate of PD-L1 positive expression grouPwas significantly lower than that of PD-L1 negative expression grouP(45.24% vs 88.89%,P<0.01), and the overall survival rate of Ki-67 positive expression grouPwas significantly lower than that of Ki-67 negative expression grouP(58.44% vs 91.67%,P<0.01). Multivariate regression analysis showed that TNM stages Ⅲ and Ⅳ, lymph node metastasis, CDX-2 negative expression, PD-L1 positive expression and Ki-67 positive expression were independent prognostic factors for patients with colorectal cancer(P<0.05). Conclusions The expression of CDX-2 is significantly down-regulated while the expressions of PD-L1 and Ki-67 are significantly up-regulated in colorectal cancer tissues. The abnormal expressions of CDX-2, PD-L1 and Ki-67 may be involved in the occurrence and development of colorectal cancer, and are closely related to the prognosis of colorectal cancer.
