乳腺癌Hpa表达与淋巴管生成的相关性

中文摘要: 目的 探讨乙酰肝素酶(Hpa)在乳腺正常组织、不典型增生、导管内癌、浸润性导管癌组织中的表达水平,分析其与浸润性乳腺癌临床病理指标及肿瘤淋巴管生成之间的关系。方法 收集西安交通大学第一附属医院2005年1月至2007年12月经手术切除的23例不典型增生组织,34例导管内癌组织,80例浸润性导管癌组织标本,以及距离肿瘤边缘5 cm以外的正常乳腺组织20例。用免疫组织化学染色法检测不同组织Hpa表达情况,分析其与浸润性乳腺癌肿瘤直径、腋窝淋巴结转移、病理分期、雌孕激素受体表达等临床病理指标的关系;并用淋巴管内皮细胞标记物D2-40标记肿瘤淋巴管,探讨Hpa表达对肿瘤淋巴管生成的影响。结果 Hpa在乳腺正常组织、不典型增生、导管内癌、浸润性导管癌组织中表达阳性率分别为15.0%、34.8%、52.9%、76.3%,呈逐渐升高趋势(χ2=31.403,P<0.01)。Hpa阳性表达率在乳腺导管内癌与浸润性癌中差异有统计学意义(χ2=6.093,P<0.05)。Hpa阳性表达率在乳腺癌不同病理分期、淋巴结转移及肿瘤直径分组中差异有统计学意义(P<0.05)。浸润性导管癌Hpa表达阳性组MLD值高于Hpa表达阴性组(9.961±4.329 vs 7.339±3.714,t=2.379,P<0.05)。Hpa表达与MLD值呈正相关关系(r=0.603,P<0.05)。结论 浸润性乳腺癌中Hpa表达水平升高,其表达上调与乳腺癌淋巴管生成相关。
Abstract:Objective To investigate the expression of Heparanase(Hpa) in normal breast tissue,atypical hyperplasia,intraductal carcinoma and invasive carcinoma,and to analyze the relationship between Hpa with clinicopathological factors and tumor lymphangiogenesis in breast cancer.Methods Twenty three cases of atypical hyperplasia tissues,34 cases of intraductal carcinoma tissues,80 cases of invasive ductal carcinoma tissue specimens,and 20 cases of normal breast tissues 5 cm away from the tumor edge after surgical removal were collected from the First Affiliated Hospital of Xi′an Jiaotong University between January 2015 and December 2007.The expressions of Hpa in different groups were detected by immunohistochemical method,and the relationship between Hpa expression and clinicopathological factors such as tumor diameter,axillary lymph node metastasis,pathological stage and expression of estrogen and progesterone receptor was analyzed.The lymphatic vessels were labeled with D2-40.The effect of Hpa expression on tumor lymphangiogenesis was analyzed.Results The positive rates of Hpa expression in normal breast tissue,atypical hyperplasia,intraductal carcinoma and invasive carcinoma were 15.0%,34.8%,52.9% and 76.3%,showed a gradually increasing trend( χ2=31.403, P <0.01).The positive expression rate of Hpa was statistically different between breast intraductal carcinoma and invasive carcinoma( χ2=6.093, P<0.05).There were significant differences in Hpa positive expression rate among groups of different clinical stages,lymph node metastasis and tumor diameter in invasive carcinoma( P<0.05).The MLD value of Hpa positive group was significantly higher than that of Hpa negative group in invasive carcinoma(9.961±4.329 vs 7.339±3.714, t=2.379, P<0.05).There was a positive correlation between Hpa expression and MLD ( r=0.603, P<0.05).Conclusions Hpa is highly expressed in invasive breast cancer,and its upregulation is related to lymphangiogenesis.
