中文摘要: 目的 评估机器人导航下髋臼前柱螺钉置入的准确性,为临床治疗提供参考。方法 回顾性收集甘肃省人民医院2022年1月至2024年1月因髋臼骨析行机器人导航前柱螺钉置入的患者,共78例,依据手术方法不同分为观察组(44例)和对照组(34例)。观察组:机器人导航下置入螺钉。对照组:透视下徒手置入螺钉。完善术后检查,依据精准性评估方法,定义优为置钉精准。统计两组前柱顺行螺钉、前柱逆行螺钉、总螺钉的精准螺钉数,比较两组患者置入螺钉精准率。结果 与对照组比较,观察组的前柱顺行螺钉精准率(92.0% vs 80.0%,χ2=0.54,P=0.462)、前柱逆行螺钉精准率(94.7% vs 85.7%,P=0.561)以及总螺钉数精准率(93.2% vs 82.4%,χ2=1.27,P=0.260)稍有增高,但差异均无统计学意义。结论 相较于透视下徒手经皮置入前后柱螺钉,机器人导航下经皮置入前后柱螺钉精准性有所增高,但差异无统计学意义。
Abstract:Objective To evaluate the accuracy of acetabular anterior column screw placement under robotic navigation,so as to provide a reference for clinical treatment. Methods A total of 78 patients who underwent anterior column screw placement with robotic navigation at Gansu Provincial Hospital from January 2022 to January 2024 were retrospectively collected. The patients were divided into observation group(44 cases)and control group(34 cases)according to different surgical methods. Screws were placed under robotic navigation in the observation group ,while screws were inserted freehand under fluoroscopic guidance in the control group. Postoperative examinations were completed. Excellent placement was defined as accurate screw position according to the accuracy evaluation criteria. The numbers of accurately placed anterograde anterior column screws,retrograde anterior column screws and total screws were counted,and the screw placement accuracy rates were compared between the two groups.Results Compared with the control group,the observation group showed slightly higher accuracy placement rates of anterograde anterior column screws(92.0% vs 80.0%,χ2=0.54,P=0.462),retrograde anterior column screws(94.7% vs 85.7% ,P=0.561),and overall screws(93.2% vs 82.4% ,χ2=1.27,P=0.260),but none of these differences were statistically significant. Conclusion Compared with freehand percutaneous screw insertion under fluoroscopy,the accuracy of percutaneous screw placement assisted by robotic navigation is somewhat higher,but the difference is not statistically significant.
