中文摘要: 目的 测定盐酸纳美芬抑制小儿全身麻醉诱导时舒芬太尼诱发呛咳(SIC)的半数有效量(ED50)及95%有效量(ED95)。方法 选择2025年11月至2026年1月南京医科大学附属儿童医院全身麻醉下行腹腔镜单侧腹股沟疝疝囊高位结扎术的患儿35例,年龄1~6岁,男29例,女6例,体质量(15.2±3.9)kg,美国麻醉医师协会(ASA)分级Ⅰ级。麻醉诱导时静脉注射盐酸纳美芬,初始剂量为0.25 μg/kg,注射完毕2 min 后静脉推注舒芬太尼0.3 μg/kg,观察1 min 后静脉注射丙泊酚2 mg/kg。观察患儿的呛咳发生情况,采用改良序贯法计算盐酸纳美芬剂量,若在注射舒芬太尼后出现阳性呛咳反应,下一例纳美芬的剂量增加0.05 μg/kg,否则降低0.05 μg/kg,当出现8 个交叉点时终止研究。采用概率单位Probit 回归分析法计算纳美芬抑制小儿麻醉诱导时SIC 的ED50、ED95及95%CI。结果 盐酸纳美芬抑制小儿麻醉诱导时SIC 的ED50为0.087 μg/kg(95%CI:0~0.168 μg/kg),ED95为0.283 μg/kg(95%CI:0.187~2.005 μg/kg)。结论 盐酸纳美芬抑制小儿全身麻醉诱导时SIC的ED50及ED95分别为0.087 μg/kg和0.283 μg/kg。小儿全身麻醉诱导时预注射0.283 μg/kg盐酸纳美芬可以有效抑制SIC。
Abstract:Objective To determine the median effective dose(ED50)and 95% effective dose(ED95)of namefene hydrochloride in inhibiting sufentanil - induced cough(SIC)during induction of general anesthesia in children.Methods Thirty-five children[aged 1-6 years,29 males,6 females,body mass(15.2±3.9)kg,American Society of Anesthesiologists physical status I],scheduled for elective laparoscopic unilateral high ligation of hernial sac under general anesthesia at the Children’s Hospital of Nanjing Medical University from November 2025 to January 2026 were enrolled. Nalmefene hydrochloride was intravenously injected at the start of anesthesia induction with an initial dose of 0.25 μg/kg. Two minutes after the injection,sufentanil 0.3 μg/kg was intravenously administered,followed by propofol 2 mg/kg one minute later. The occurrence of cough was observed. The dose of nalmefene was determined by a modified sequential method:if a positive cough response occurred after sufentanil injection,the next patient’s dose was increased by 0.05 μg/kg;otherwise,it was decreased by 0.05 μg/kg. The study was terminated after eight cross-over points were achieved. The ED50,ED95 and 95% CI of nalmefene for inhibiting SIC during anesthesia induction in children were calculated using Probit regression analysis. Results The ED50 and ED95 of nalmefene in inhibiting SIC during anesthesia induction were 0.087 μg/kg(95% CI:0- 0.168 μg/kg)and 0.283 μg/kg(95% CI:0.187- 2.005 μg/kg),respectively. Conclusion The ED50 and ED95 of nalmefene for prevention of SIC during anesthesia induction in children Fig.1 Sequential diagram of nalmefene inhibiting SIC in children were 0.087 μg/kg and 0.283 μg/kg,respectively. Pretreatment with 0.283 μg/kg of nalmefene hydrochloride can effectively prevent SIC during induction of general anesthesia in children.
