老年游离端缺失致颞下颌关节紊乱患者关节盘与关节囊的变化

中文摘要: 目的 通过磁共振成像(MRI)探索中老年游离端缺失致颞下颌关节紊乱(TMD)患者关节盘与关节囊的变化,为TMD的诊疗提供一定的理论依据。方法 本研究选取了2021至2023年于南京医科大学第四附属医院就诊的63例中老年TMD患者的MRI,根据临床检查中游离端缺失情况分为单侧缺失组(n=21)、双侧缺失组(n=21)以及未缺失组(n=21),比较三组患者的颞下颌关节的关节盘与关节囊的变化。结果 单因素方差分析表明三组间关节盘高度、关节盘面积、关节囊前囊面积、关节囊后囊高度、关节囊后囊面积差异有统计学意义(P<0.05)。未缺失组与单侧缺失组间关节盘面积、关节囊前囊高度差异具有统计学意义(P<0.05)。双侧缺失组的关节盘高度以及关节盘、关节囊前囊、关节囊后囊的面积均显著低于未缺失组或单侧缺失组(P<0.05)。结论 双侧游离端缺失可能导致TMD患者关节盘高度降低和面积缩小,并导致关节囊面积缩小。
Abstract:Objective To investigate the changes of the articular disc and capsule in middle-aged and elderly patients with temporomandibular disorders(TMD)induced by distal-extension edentulism using magnetic resonance imaging, and to provide a theoretical basis for the diagnosis and treatment of TMD. Methods In this study, the MRI images of63 middle-aged and elderly patients with TMD who were treated in the Fourth Affiliated Hospital of Nanjing Medical University from 2021 to 2023 were selected, and the patients were divided into unilateral-defect group, bilateral-defect group, and non-defect group based on the distal-extension edentulism in clinical examination, and the changes of the joint disc and joint capsule of the temporomandibular joint of the three groups were compared. Results One-way ANOVA showed that there were significant differences in height and area of articular disc, area of anterior capsule, height and area of articular disc posterior capsule among three groups(P<0.05). There were significant differences in area of articular disc and height of anterior capsule between the non-defect group and the unilateral-defect group(P<0.05).The height and the areas of the articular disc, anterior joint capsule, and posterior joint capsule in the bilateral-defect group were significantly lower than those in the non-defect group or the unilateral-defect group. Conclusion Double distal-extension edentulous may lead to a decrease in the height and area of the joint disc in patients with TMD, and lead to a decrease in the area of the joint capsule.
