新生儿持续肺动脉高压血清CX3CL1、Gal-3水平及与病情、预后的关系

中文摘要: 目的 探究新生儿持续肺动脉高压(PPHN)血清趋化因子CX3C基元配体1(CX3CL1)、半乳糖凝集素-3(Gal-3)水平及与病情、预后的关系,为PPHN病情评估和预后预测提供参考。方法 选择2021年2月至2024年12月山西医科大学第二医院收治的196例继发性PPHN新生儿为研究对象(病例组),根据病情严重程度分为重度组(65例)、中度组(71例)、轻度组(60例)。另选择196例同期足月分娩的正常新生儿作为对照组。采用酶联免疫吸附法检测其血清CX3CL1、Gal-3水平。采用Spearman相关系数分析血清CX3CL1、Gal-3水平与PPHN患儿病情严重程度的关系;采用多因素logistic回归模型分析影响PPHN患儿30 d内生存状况的因素;采用受试者工作特征(ROC)曲线评价血清CX3CL1、Gal-3对PPHN患儿预后的预测价值。结果 病例组血清CX3CL1、Gal-3水平均高于对照组(P<0.05)。患儿血清CX3CL1、Gal-3水平依重度组、中度组、轻度组之序递(P<0.05),且分别与PPHN病情严重程度呈正相关(r=0.516,0.493,P<0.05)。死亡组(33例)患儿氧合指数≥16的占比以及血清CX3CL1、Gal-3水平显著高于生存组(163例)(P<0.01)。氧合指数≥16及血清CX3CL1、Gal-3水平升高均是导致 PPHN 患儿 30 d 内死亡的危险因素(P<0.05)。血清CX3CL1、Gal-3联合预测 PPHN 患儿预后的曲线下面积(AUC)为0.873(95%CI:0.791~0.931),敏感度为90.91%,特异度为80.60%,二者联合检测的AUC高于CX3CL1、Gal-3单独预测的AUC(Z=2.159,P=0.032;Z=2.037,P=0.045)。结论 PPHN患儿血清CX3CL1、Gal-3水平升高,与患儿病情严重程度呈正相关,二者联合检测对患儿30 d预后具有较高的预测价值。
Abstract:Objective To investigate the levels of serum chemokine CX3C ligand 1(CX3CL1)and galectin-3(Gal-3)in persistent pulmonary hypertension of newborn (PPHN) and their relationship with the disease condition and prognosis,and to provide a reference for the assessment and prognostic prediction of PPHN. Methods A total of 196 newborns with secondary PPHN admitted to the Second Hospital of Shanxi Medical University from February 2021 to December 2024 were regarded as the study subjects(case group). And the newborns were separated into severe group(n=65),moderate group(n=71),and mild group(n=60)according to the severity of the disease condition. Another 196 normal newborns who were delivered at full term during the same period were regarded as the control group. Enzyme-linked immunosorbent assay(ELISA)was applied to detect serum levels of CX3CL1 and Gal-3. Spearman correlation coefficient was applied to analyze the relationship between serum levels of CX3CL1 and Gal-3 and the severity of PPHN in infants. Multivariate logistic regression model was applied to analyze the factors affecting the prognosis of newborns with PPHN. Receiver operating characteristic(ROC)curve was applied to evaluate the predictive value of serum CX3CL1 and Gal-3 for the prognosis of infants with PPHN. Results The serum levels of CX3CL1 and Gal-3 in the case group were higher than those in the control group(P<0.05). The serum levels of CX3CL1 and Gal-3 decreased sequentially in the severe,moderate,and mild groups(P<0.05). Serum levels of CX3CL1 and Gal-3 were positively correlated with the severity of PPHN in newborns(r=0.516,0.493,P<0.05). The proportions of oxygenation index≥16,serum CX3CL1 and Gal-3 levels in the death group(n=33)showed significant differences compared to the survival group (n=163)(P<0.01). An oxygenation index ≥16 and elevated serum levels of CX3CL1 and Gal-3 were independent risk factors for 30-day mortality in infants with PPHN(P<0.05). The area under the curve(AUC)of serum CX3CL1 and Gal-3 in combination for predicting the prognosis of newborns with PPHN was 0.873(95%CI:0.791-0.931),with a sensitivity of 90.91% and a specificity of 80.60%. The AUC of combined detection of the two was higher than that of CX3CL1 and Gal-3 alone(Z=2.159,P=0.032;Z=2.037,P=0.045). Conclusion The elevated levels of serum CX3CL1 and Gal-3 in newborns with PPHN are positively correlated with the severity of condition,and the combined detection of these two has high predictive value for the 30-day prognosis of newborns.
