Abstract:Objective To explore the clinical efficacy of hemodialysis combined with hemoperfusion and its effects on the levels of serum parathyroid hormone（PTH）and 25 hydroxyvitamin D3［25（OH）D3］in maintenance hemodialysis patients with diabetic nephropathy（DN），so as to provide a reference for clinical practice. Methods A total of 100 maintenance hemodialysis patients with DN treated at Yancheng First People's Hospital from April 2023 to April 2024 were enrolled. They were randomly divided into an observation group（n=50，treated with hemodialysis combined with hemoperfusion）and a control group（n=50，treated with hemodialysis alone）. The efficacy，renal function，25（OH）D3，PTH，inflammatory indicators，and untoward reactions of the two groups were compared. Results The total effective rate of the observation group was higher than that of the control group（98.00% vs 82.00%，χ2=7.111，P=0.008）. After treatment，blood urea nitrogen（BUN），serum creatinine（Scr），PTH，high-sensitivity C-reactive protein（hs-CRP），and interleukin-6（IL-6）all decreased in both groups，while 25（OH）D3 level increased，with the observation group showing better changes（P<0.05）. There was no obvious difference in adverse reactions between the observation group and the control group（12.00% vs 18.00%，χ2=0.706，P=0.401）. Conclusion Hemodialysis combined with hemoperfusion has obvious therapeutic effects on maintenance hemodialysis patients with DN. The combination can improve renal function，optimize PTH and 25（OH）D3 levels，and reduce inflammatory reactions，with high safety.
