中文摘要: 帕金森病(PD)是一种以运动障碍为典型特征的进行性神经退行性疾病,但其非运动症状,特别是抑郁,严重影响患者生活质量且常早于运动症状出现。近年来,神经免疫炎症在PD及其抑郁症状发病机制中的作用备受关注。本文系统综述了PD状态下神经炎症微环境的特征,重点探讨小胶质细胞活化、线粒体功能障碍与多巴胺能神经元损伤的相互作用。在此基础上,分析本文G蛋白偶联受体109A(GPR109A)作为连接代谢与免疫的关键分子,在调控神经炎症中的潜在价值及其在PD治疗中的前景。最后,结合内侧前额叶皮质在抑郁发生中的解剖学基础,本文提出了GPR109A激动剂可能通过改善神经炎症从而缓解PD抑郁症状的假说,为未来研究提供新思路。
Abstract:Parkinson’s disease (PD) is a progressive neurodegenerative disorder classically characterized by motor symptoms. However, its non-motor symptoms, particularly depression, significantly impact patients’ quality of life and often precede the onset of motor symptoms. In recent years, the role of neuroimmune inflammation in the pathogenesis of PD and its associated depressive symptoms has garnered increasing attention. This article systematically reviews the characteristics of the neuroinflammatory microenvironment in the context of PD, with a focus on the interplay between microglial activation, mitochondrial dysfunction, and dopaminergic neuron damage. Based on this, this article analyzes the potential value of the G protein coupled receptor 109A (GPR109A) receptor, a key molecule linking metabolism and immunity, in regulating neuroinflammation and its prospects for PD therapy. Finally, considering the anatomical basis of the medial prefrontal cortex in the pathogenesis of depression, this article proposes the hypothesis that GPR109A agonists may alleviate depressive symptoms in PD by ameliorating neuroinflammation, thereby offering new insights for future research.
