中文摘要: 目的 探讨糖尿病早期肾损伤患者血肾损伤分子-1(KIM-1)、尿微量白蛋白与尿肌酐比值(UACR)、脂代谢变化及预测价值,为糖尿病患者早期肾损伤的预测提供新思路。方法 选择2022年6月至2024年6月联勤保障部队第九八二医院和石家庄市第二医院收治的160例糖尿病患者,根据是否合并肾损伤分为合并肾损伤组(n=80)与单纯糖尿病组(n=80),比较两组KIM-1、UACR以及脂代谢指标,采用多因素logistic回归分析影响肾损伤的因素。绘制受试者工作特征曲线(ROC)分析 KIM-1、UACR 及脂代谢指标对糖尿病早期肾损伤的预测效能。结果 合并肾损伤组KIM-1、UACR、TG水平及高血压占比、糖尿病病程高于单纯糖尿病组(P<0.05)。多因素logistic回归分析结果显示,KIM-1(OR=3.647,95%CI:1.239~10.740)、UACR(OR=2.726,95%CI:1.109~6.704)、TG(OR=3.043,95%CI:1.085~8.533)为糖尿病患者早期肾损伤的独立影响因素(P<0.05)。ROC曲线结果显示,KIM-1、ACR、TG预测糖尿病患者早期肾损伤的曲线下面积(AUC)分别为0.896、0.838、0.702,其中KIM-1的AUC最高,敏感度、特异度分别为81.25%、92.50%。结论 KIM-1、ACR及TG为糖尿病患者早期肾损伤的影响因素,均对糖尿病早期肾损伤具有较好的预测效能,其中以KIM-1的预测效能最佳。
Abstract:Objective To explore the changes and predictive value of serum kidney injury molecule-1(KIM-1),urinary microalbumin-to-creatinine ratio(UACR),and lipid metabolism in patients with diabetes and early kidney injury,and to provide new ideas for the prediction of early kidney injury in diabetic patients. Methods From June 2022 to June 2024,160 patients with diabetes admitted to the 982nd Hospital of the Joint Logistics Support Force and the Second Hospital of Shijiazhuang were selected as study subjects,and assigned to the kidney injury group(n=80)and the simple diabetes group(n=80)according to whether they had kidney injury. KIM-1,UACR and lipid metabolism indicators were compared between the two groups. The influencing factors were screened by multivariate logistic regression analysis. The receiver operating characteristic(ROC)curves were plotted to analyze the predictive efficacy of KIM-1,UACR,and lipid metabolism indicators for early kidney injury in diabetes. Results The levels of KIM-1,UACR and triglyceride(TG),as well as the proportion of hypertension and the duration of diabetes in the kidney injury group were higher than those in the simple diabetes group(P<0.05). Multivariate logistic regression analysis showed that KIM-1(OR=3.647,95%CI:1.239-10.740),UACR(OR=2.726,95%CI:1.109-6.704),and TG(OR=3.043,95%CI:1.085-8.533)were influencing factors of early kidney injury in diabetes(P<0.05). ROC curve analysis results showed that the areas under the curve(AUC)of KIM-1,ACR,and TG to predict early kidney injury in diabetes were 0.896,0.838,and 0.702,respectively. Among them,KIM-1 had the highest AUC,with a sensitivity of 81.25% and a specificity of 92.50%. Conclusion KIM-1,UACR,and TG are influencing factors of early kidney injury in diabetes. All three indicators have good predictive efficacy for early kidney injury in diabetes,and KIM-1 shows the best predictive performance.
