中文摘要: 目的 对比分析短臂管型和长臂支具固定治疗下尺桡关节稳定的三角纤维软骨复合体(TFCC)损伤的效果差异。方法 回顾性分析2023年1月至12月无锡市第九人民医院收治的下尺桡关节稳定的TFCC损伤采取支具固定保守治疗的51例患者的临床资料,其中短臂腕关节中立位管型支具固定21例(短臂组),过肘和腕的半旋后位长臂支具固定30例(长臂组),治疗方案为短臂组短臂支具固定6周,长臂组长臂支具固定3周后、短臂支具再固定3周。所有患者拆除支具后接受系统康复治疗。评价指标包括疼痛视觉模拟量表(VAS)评分、关节活动度、握力、改良Mayo腕关节评分、患者自我评价腕关节评估量表(PRWE)评分。结果 平均随访14.2个月。至末次随访时,两组患者疼痛评分、握力、旋转活动度、腕关节功能评分较治疗前均显著改善(P<0.05),其中治疗3个月时的短臂支具组旋转活动度优于长臂组(P<0.05)。结论 短臂和长臂支具固定均可有效治疗下尺桡关节稳定的TFCC损伤,且短臂支具可在早期更快恢复旋转功能。
Abstract:Objective To compare and analyze the efficacy differences between short-arm cast brace fixation and long-arm brace fixation in the treatment of triangular fibrocartilage complex(TFCC)injuries with stable distal radioulnar joint. Methods A retrospective analysis was conducted on the clinical data of 51 patients with TFCC injuries and stable distal radioulnar joint who received conservative treatment with brace fixation in the Wuxi Ninth People’s Hospital from January to December 2023. Among them,21 cases were treated with short-arm wrist cast brace fixation in neutral position(short-arm group),and 30 cases were treated with long-arm brace fixation in supinated position over elbow and wrist(long-arm group). The treatment protocol was 6 weeks of short-arm brace fixation in the short-arm group,and 3 weeks of long-arm brace fixation followed by 3 weeks of short-arm brace fixation in the long-arm group. All patients received systematic rehabilitation treatment after brace removal. The evaluation indicators included Visual Analogue Scale(VAS)score for pain,joint range of motion,grip strength,modified Mayo Wrist Score(MMWS),and Patient-Rated Wrist Evaluation(PRWE)score. Results The average follow-up period was 14.2 months. At the final follow-up,the pain score,grip strength,rotational range of motion,and MMWS and PRWE scores of both groups were significantly improved compared with those before treatment(P<0.05). Among them,the rotational range of motion in the short-arm group was better than that in the long-arm group at 3 months after treatment(P<0.05). Conclusion Both short-arm and long-arm brace fixation can effectively treat TFCC injuries with stable distal radioulnar joint,and short - arm brace can restore rotational function faster in the early stage.
