中文摘要: 目的 探讨改良双垫矫治器（twin-block矫治器）联合引导组织再生术（GTR）治疗Ⅱ类错畸形伴牙周
炎患者的美学效果及对牙周健康、牙齿功能的影响。方法 选取2021年1月至2024年1月武汉市第一医院收治的98例Ⅱ类错畸形患者作为研究对象，根据治疗方案不同分成对照组（单纯GTR，n=49）和联合组（GTR+改良twin-block矫治器，n=49）。比较两组临床疗效、美学效果［正畸需求治疗指数的美学成分量表（IOTN-AC）］、牙周健康指标［牙周附着丧失度（AL）、菌斑指数（PLI）、探诊深度（PD）］、牙齿功能［咬合力总值（TOF）、咬合力中心点位置（COF）、咬合力中心点最大位移（MMCOF）、咀嚼效率］及治疗满意度。结果 联合组的临床疗效总有效率为91.84%（45/49），高于对照组的75.51%（37/49）（χ2=4.780，P=0.029）。治疗后，两组IOTN-AC评分、AL、PLI、PD均降低（P<0.05），且联合组IOTN-AC评分、AL、PLI、PD低于对照组（P<0.05）。治疗后两组TOF和咀嚼效率升高，COF、MMCOF降低（P<0.05）；且联合组TOF水平和咀嚼效率高于对照组，COF、MMCOF水平低于对照组（P<0.05）。联合组治疗满意度总得分为（91.23±6.57）分，高于对照组的（87.62±7.36）分，差异有统计学意义（t=2.561，P=0.012）。结论 改良twin-block矫治器联合GTR能有效提升Ⅱ类错畸形伴牙周炎患者的美学效果，改善牙周健康和牙齿功能，疗效优于单纯应用GTR，患者的治疗满意度良好。
Abstract:Objective To explore the aesthetic effect and impact on periodontal health and tooth function of modified twin-block appliance combined with guided tissue regeneration (GTR) in the treatment of patients with Class Ⅱ malocclusion and periodontitis. Methods A total of 98 patients with Class Ⅱ malocclusion admitted to Wuhan First Hospital from January 2021 to January 2024 were selected as the research subjects. According to different treatment plans, the patients were divided into a control group (GTR, n=49) and a combined group (GTR+modified twin-block appliance, n=49). The clinical efficacy, aesthetic effects [Aesthetic Component of the Index of Orthodontic Treatment Need (IOTN-AC)], periodontal health indicators [attachment loss (AL), plaque index (PLI), probing depth (PD)], tooth function [total occlusal force (TOF), center occlusal force (COF), maximum mandibular central occlusal force (MMCOF), chewing efficiency]and treatment satisfaction were compared between the two groups. Results The total clinical efficacy rate of the combination group was 91.84% (45/49), which was higher than that of the control group [75.51% (37/49)] (χ2=4.780, P=0.029). After treatment, IOTN-AC score, AL, PLI, and PD were all reduced in both groups (P<0.05), and IOTN-AC score, AL, PLI, and PD in the combined group were lower than those in the control group (P<0.05). After treatment, TOF and chewing efficiency of the two groups increased, and COF and MMCOF decreased (P<0.05), and the TOF level and chewing efficiency of the combined group were higher than those of the control group, COF and MMCOF levels were lower than those in the control group (P<0.05). The total treatment satisfaction score of the combined group was 91.23±6.57, which was higher than that of the control group (87.62± 7.36), and the difference was statistically significant (t=2.561, P=0.012). Conclusion The modified twin-block appliance combined with GTR can effectively improve the aesthetics of patients with Class Ⅱ malocclusion and periodontitis, improve periodontal health and tooth function. The therapeutic efficacy is better than that of GTR alone, and the patient's treatment satisfaction is good.
