中文摘要: 目的 探讨慢性丙型肝炎(CHC)患者血清中可溶性程序性死亡因子1(sPD-1)和配体(sPD-L1)水平变化与患者病毒载量及肝纤维化程度的关系。方法回顾性选取2022年10月至2023年10月70例在南京医科大学第四附属医院感染科门诊就诊的CHC患者为研究组,另选取同期70名体检的健康者为对照组。采用酶联免疫吸附法检测各组血清sPD-1和sPD-L1水平,采用Pearson相关法分析CHC患者血清中sPD-1和sPD-L1水平与疾病活动性指标[丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)]、丙型肝炎病毒载量指标(HCV RNA)和肝纤维化指标[AST/血小板指数(APRI)]的相关性。结果与健康对照组相比,CHC组患者血清sPD-1、sPD-L1、ALT和AST水平显著升高(P <0.01)。Pearson相关性分析结果显示,CHC组血清sPD-1水平与ALT、AST及HCV RNA水平分别呈正相关(P<0.05);APRI与血清sPD-1水平呈正相关(P<0.05),且APRI高水平(≥1.2)组患者的血清sPD-1水平明显高于APRI低水平组(≤0.42),差异有统计学意义(P<0.05)。结论在CHC患者中,sPD-1水平可能是反映疾病活动性、病毒载量和肝纤维化程度的重要指标,而sPD-L1在疾病中的具体作用机制则仍需更多研究加以明确。
Abstract:Objective To investigate the changes in serum levels of soluble programmed cell death protein 1(sPD-1)and its ligand(sPD-L1)in patients with chronic hepatitis C(CHC)and their relationship with hepatitis C viral load(HCV RNA)and degree of liver fibrosis. Methods A total of 70 CHC patients who visited the Infectious Disease Outpatient Clinic of the Fourth Affiliated Hospital of Nanjing Medical University from October 2022 to October 2023 were respectively selected as the study group,and 70 healthy individuals who underwent physical examination during the same period were selected as the control group. Serum levels of sPD-1 and sPD-L1 were measured by enzyme-linked immunosorbent assay(ELISA). The correlations of serum levels of sPD-1 and sPD-L1 with disease activity indicators[alanine aminotransferase(ALT),aspartate aminotransferase(AST)],hepatitis C viral load indicator(HCV RNA)and liver fibrosis indicator[AST/platelet index(APRI)]were analyzed by Pearson correlation. Results Compared to the control group,CHC patients had significantly elevated serum levels of sPD-1,sPD-L1,ALT,and AST,with statistically significant differences(P <0.01). Pearson correlation analysis showed that serum sPD-1 levels in the CHC group were positively correlated with ALT,AST,and HCV RNA,respectively(P<0.05). Moreover,APRI was positively correlated with serum sPD-1 levels(P<0.05),with patients in the high APRI(≥1.2)group displaying significantly higher serum sPD-1 level than that in the low APRI(≤0.42)group(P<0.05). Conclusion The level of sPD-1 in CHC patients may be served as an important indicator reflecting the disease activity,viral load,and the degree of liver fibrosis,while the specific mechanism of sPD-L1 in disease progression requires further investigation.
