中文摘要: 抗菌药物的心脏毒性已成为近些年预防和治疗心脏瓣膜疾病(valvular heart disease,VHD)的关注方向之一。急性瓣膜功能障碍主要与药物剂量或过敏有关,慢性瓣膜病变以瓣膜退行性变为特点。这种损伤的危险因素较多,包括高龄、人工瓣膜置换、合并肾功能不全,其机制包括胶原蛋白代谢紊乱及药物蓄积性毒性增加等。本文通过综述抗菌药物致心脏瓣膜的不良反应的研究现状,从常用的抗菌药物类型、损伤机制、临床表现及其预防措施方面,探讨不同抗菌药物对心脏瓣膜的损伤,为临床中兼顾预防感染的治疗效果和心脏安全提供循证依据。
Abstract:In recent years,the cardiotoxicity of antimicrobials has become a focus in the prevention and treatment of valvular heart disease(VHD). Acute valve dysfunction is primarily associated with drug dosage or allergic reactions,while chronic valve disease is characterized by valve degeneration. The risk factors contributing to this type of damage are multiple,including advanced age,prosthetic valve replacement,and renal dysfunction. The underlying mechanisms involve disorders of collagen metabolism and increased cumulative drug toxicity. This article provides a comprehensive review of current research on the adverse effects of antimicrobials on cardiac valves,examining the impact of various antimicrobials from perspectives such as types of antimicrobials,injury mechanisms,clinical manifestations,and preventive measures. The goal is to offer an evidence-based foundation for balancing infection prevention and cardiac safety in clinical practice.
