中文摘要: 目的 探讨颈椎磁共振T2轴位像评估颈椎椎管狭窄程度的临床价值,以指导临床精准诊疗。方法 选取2021年6月至2024年6月安徽医科大学第三附属医院收治的80例颈椎病患者的临床资料。均采用核磁共振仪检测患者颈椎椎管,并进行图像分析,在颈椎T2轴位像磁共振成像上找到C3~C7各节段压迫最严重的层面测量椎管有效矢状径(ESSC)。分析C3~C7各节段不同程度颈椎椎管狭窄患者的ESSC,采用Spearman相关法分析C3~C7 各节段分级与颈椎椎管狭窄程度的相关性。结果 80 例椎管狭窄患者经影像学检查显示:0 级 34 例(42.50%),1级26例(32.50%),2~3级20例(25.00%)。颈椎椎管狭窄0级患者ESSC平均值为(10.62±1.12)mm,1级患者ESSC平均值为(9.41±0.97)mm,2~3级患者ESSC平均值为(8.59±0.93)mm。随着颈椎椎管狭窄程度的增加,ESSC 值逐渐下降(F=26.302,P<0.01)。在 C3~C7 各节段中,ESSC 值与颈椎椎管狭窄程度呈正相关(P<0.05)。结论 颈椎磁共振T2轴位像中ESSC值随着颈椎椎管狭窄程度增加而逐渐减小,具有一定应用价值。
Abstract:Objective To explore the clinical value of cervical magnetic resonance T2 axial image in evaluating thedegree of cervical canal stenosis,in order to guide precise clinical diagnosis and treatment. Methods The clinical data of 80 patients with cervical spondylosis admitted to the Third Affiliated Hospital of Anhui Medical University from June 2021 to June 2024 were selected. All patients underwent magnetic resonance imaging(MRI)to detect the cervical spinal canal,and image analysis was performed. The effective sagittal spinal canal(ESSC)of the spinal canal was measured by identifying the most severely compressed planes in the C3-C7 segments on the T2 axial MRI of the cervical spine. The ESSC of patients with varying degrees of cervical spinal stenosis in each segment of C3-C7 was analyzed,and Spearman correlation analysis was performed to examine the correlation between the grading of each segment of C3-C7 and the degree of cervical spinal stenosis. Results Imaging examination of 80 patients with spinal canal stenosis showed that 34 cases(42.50%)were grade 0,26 cases(32.50%)were grade 1,and 20 cases(25.00%)were grade 2-3. The average ESSC value of patients with grade 0 cervical spinal stenosis was(10.62±1.12)mm,grade 1 patients was(9.41±0.97)mm,and grade 2-3 patients was(8.59±0.93)mm. As the degree of cervical spinal stenosis increased,the ESSC value gradually decreased(F=26.302,P<0.01). In the C3 to C7 segments,the ESSC values were positively correlated with the degree of cervical spinal stenosis(P<0.05). Conclusion The ESSC values in the T2 axial image of cervical magnetic resonance gradually decrease with the degree of cervical spinal canal stenosis,which have certain practical value.
