中文摘要: 目的 调查脑卒中吞咽障碍患者早期肠内营养不耐受发生现状,并分析其影响因素,以期帮助脑卒中吞咽障碍患者优化营养支持策略、改善其预后情况。方法 选取2022年1月至2023年8月在内蒙古医科大学附属医院诊治的260例脑卒中吞咽障碍患者,调查分析其早期肠内营养不耐受发生现状,根据患者是否发生喂养不耐受将其分为不耐受组(n=67)和耐受组(n=193),比较两组患者一般资料,并采用logistic回归分析影响脑卒中吞咽障碍患者早期肠内营养不耐受发生的独立危险因素。结果 脑卒中吞咽障碍患者肠内营养不耐受总发生率为25.77%(67/260),其中腹泻发生率最高为8.46%,其次为腹胀,发生率为5.77%,再次为反流、胃潴留,发生率为4.23%,最后为呕吐和便秘,发生率分别为3.46%和2.31%。Logistic回归分析结果显示,患者年龄≥60岁(OR=4.141,95%CI=2.021~8.486)、肠道感染(OR=2.713, 95%CI=1.327~5.548)、亚低温治疗(OR=4.289,95%CI=2.093~8.788)为影响脑卒中吞咽障碍患者早期肠内营养不耐受的独立危险因素(P<0.05)。结论 脑卒中吞咽障碍患者早期肠内营养不耐受发生和患者年龄较大、肠道感染和亚低温治疗情况有关。
Abstract:Objective To investigate current status of early enteral nutrition intolerance in patients with post- stroke dysphagia and analyze its influencing factors,and to optimize nutritional support strategies and improve clinical outcomes for post-stroke patients with dysphagia. Methods A total of 260 patients with post-stroke dysphagia in the Affiliated Hospital of Inner Mongolia Medical University were enrolled as the research objects from January 2022 to August 2023. The current status of early enteral nutrition intolerance was investigted. According to presence or absence of feeding intolerance,patients were divided into intolerance group(n=67)and tolerance group(n=193). The general data of the two groups were compared. The independent risk factors of early enteral nutrition intolerance were analyzed by logistic regression analysis. Results The total incidence of enteral nutrition intolerance in patients with post-stroke dysphagia was 25.77%(67/260). In all types of intolerance,incidence of diarrhea was the highest(8.46%),followed by abdominal distension(5.77%),reflux and gastric retention(4.23%),vomiting(3.46%)and constipation(2.31%).The results of logistic regression analysis showed that age ≥60 years(OR=4.141,95% CI=2.021- 8.486),intestinal infection (OR=2.713,95% CI=1.327- 5.548) and mild hypothermia therapy (OR=4.289,95% CI=2.093- 8.788) were independent risk factors of early enteral nutrition intolerance(P<0.05). Conclusion The occurrence of early enteral nutrition intolerance is related to advanced age,intestinal infection and mild hypothermia therapy in patients with post-stroke dysphagia.
